"I think its an APAP that is set for 4-20"

Many of us experienced ripping our masks off during the night the first weeks. We required a break-in period!
A bottom setting of 4 or even 5 may make you feel like you are suffocating....I would try 6 or 7.
The really low setting would also cause you to experience a wide range of pressure changes that may disturb you.
To determine what is really going on you should invest in the report software available for your machine.
Get it at cpap.com and they can overnight it to you.
You may have serious mask leaks, or other problems it can help you identify.
Do some serious research on this site, order your software, and you will be able to take charge of your therapy and make rapid progress.

You will do fine.

My machine does have a ramp time of 20 minutes. I can't adjust it, though. If I could I would make it 30-40. I suppose I can turn the machine off and then on, that would work, but by doing that I might be wide awake again. I still can't sleep with the machine, but on my 3rd night I felt like maybe I could if only I was a little more tired. This seems like progress to me. This weekend I should be able to do better.

Thanks everyone for the feedback, this is helping me much more than the people who are actually paid to help me.

Update: I found this link which may be a source for your software:

This may be all you need, however I'm not familiar with your machine so research it carefully before you purchase anything:

This maybe a little late (but joined this forum)

I have the same problem of exhalation under pressure. I tried the Resmed Autoset Spirit machine first, then a Resmed VPAP IV (Bilevel) - I had problems with both. In the Autoset Spirit I had problems breathing out against the pressure, in the VPAP IV (EPAP=6; IPAP=12) was more comfortable but after about 75% of my exhalation cycle would complete the machine would increase the pressure making it uncomfortable. To be fair to the VPAP IV machine I did notice when I awoke in the middle of the night my breathing had syncronized with the machines timings. Then I tried the Respironics Auto M with A-Flex - for the first time after 3 nights I was able to sleep soundly, I noticed the pressure had gone as high as 18 and 20 but I had no appreciable discomfort in exhalation. I used this machine on 4 nights. Now I am trying out the DeVilbas RPM Bilevel - this one also is excellent and extremely comfortable.

I am lucky my doctor has arranged for me to try out several machines before deciding which one to buy.
