Just started using Resmed AirSense 10 Autoset - help with Oscar data

Hi all!

Was using a Z1 travel CPAP for the last 9 months or so, and have had pretty good luck with it. I was on 13.5 for the pressure, which is about as high as I could go without being uncomfortable, or really having to have the mask TIGHT to prevent leaks.

I just upgraded to the Airsense 10 Autoset, and I think I have things set pretty well, but it's now 3-4 nights in, and I'm pretty tired during the last few days. I'm new to Oscar, but have been a lurker here from time to time, and I think it's amazing we can pull that much data.

Anyways - not sure if there will be a glaring issue or not. The one thing I've noticed, is that with my old Z1, I could hear the air/pressure going through my Dreamwear headgear. With the Airsense, I don't hear anything at all.. which made me think I'm just not getting the pressure or volume. But when I pull the mask off a touch just to get it to leak, there's a TON of air there..

Settings on the Airsense look like they're in the Oscar report, but let me know if you have any questions so you can help diagnose.

Hello, mjrodman. You have a great machine there. One of its virtues is how quiet it is. Not sure why you'd also hear a difference in the mask channels, but in your place I'd count that as a plus!

I think it's probably pretty common to sleep a little less well when we change from one machine to another. We just get used to a certain feel, and when that changes, we're on higher alert for a little bit.

Your machine has the capacity to drop your pressure when you exhale, something it can do very smoothly. It's basically a comfort feature, but it might also help reduce your flow limitations and hypopneas. Reducing them might make no difference to the quality of your sleep, but then again it might make some difference.

At any rate, I'd suggest that you introduce EPR, starting with a level of 1. Normally the suggestion would be to raise your minimum by 1, but your obstructive apneas look very well controlled, so it might make sense to leave the minimum where it is and see how that goes.

Here's last night's.. I feel a little better today.

I upped the min pressure to 13 which is more inline with my previous CPAP pressure. I'm still not sure why I'm not noticing the pressure/noise as much with the Airsense. Perhaps because my Z1 was a smaller motor, spinning faster/louder is why.. not sure.

Yeah, my previous CPAP had that ability too, and honestly I can't imagine NOT having that flow with the inhale/exhale. That would be uncomfortable!

EDIT - Oh, I thought that was already happening.. this CPAP is pretty amazing for comfortability if that's not even happening. That's what I'm kinda saying, I don't at ALL notice the pressure compared to my other machine which had the EPR already turned on.. WOW.

OK, I'll turn it on a little bit tonight, and see how that goes.

Also, I've finally taken some time to ingest the two Oscar outputs above, and looks like the ramp up time is extremely long at ~2 hours. I'm going to decrease that to about 20-30 mins I think.

One more thing I think I'm noticing above from the pressure is that it's correlating with events. Looks to me like the pressure is ramping up, and when it gets higher than my body likes, it causes an event, which I'm guessing is a wake-up of sorts. I think what I'm gleaning from this is that my high pressure should be just below where it's been causing events.. so maybe 15.5.

I can't see the flow limitation graph on your most recent chart, but on the earlier one, the FLs were the main driver for the pressure changes. In addition to that, except for one RERA, the pressure tends to increase in response to an event, i.e., after the event.

I think there's little reason for you to be increasing your minimum. Let's see what happened when you introduced EPR.

I'll post tonight's sleep tomorrow. I adjusted the humidity yesterday and ended up with condensation in the tube. I've backed that off, but left the EPR. Will post it up tomorrow.

Very nice! Stay with these settings for a week and keep track of how you are feeling during the day. Once there are 7-8 nights to go on, we could reassess.

Also, I've recently purchased the Resmed Airsense 10 Autoset. it's very comfortable and has better AHI per night, but shows very less info compared to my previous Philips ReMaster device.
