Looking for advise on settings - wife says I'm snoring

Post by Bonder » Fri Oct 21, 2022 8:19 pm

Hello,

I have been on CPAP for a long time now, probably getting close to 20 years, if not longer. My wife says I have been snoring lately and I am not feeling as refreshed as I used to, but my AHI number is still low. I was hoping to get some extra eyes on my reports to see if someone else can see something that I don't

Thanks in advance.

Looks like your minimum pressure is too low now.
I say this because every time your APAP decide's "all is well, we can lower the pressure" flow limitations appear, and the pressure has to shoot back up.
You don't show us the snore graph - but I bet snores are accompanying those flow limitations.
The aim is to get the minimum pressure high enough to prevent these flow limitations and snores.

AHI isn't everything....
Flow limitations and snores mean your airway is partially collapsing - probably would have resulted it total obstructive apneas if your machine hadn't raised the pressure.

To follow up on Ozij’s post, try raising your minimum by 2 and turn on EPR to 1. If that feels OK to you, then increase EPR to 2 and then to 3. The EPR drops your pressure when you exhale; by the same token it increases your pressure when you inhale, which can help with flow limitations.

Thanks for the advice so far. I will try bumping the pressure tonight (What I was thinking as well). I will also try the EPR. I have never used it before, but its function makes sense to me.

To follow up on the EPR comment, would that be a full time EPR or Ramp only? I don't use ramp now, but I could set it to full pressure and then use auto to turn it off. I feel like I can't breathe when I tried ramp in the past.

If you don't use the ramp then set EPR to full time.

I will throw my two cents in, however: When you're starting to dial wing, it's useful to change only one thing at a time. That way if anything goes wrong (the AHI shoots up or you feel super uncomfortable trying to get to sleep), you know what to back off.

I'd suggest bumping your min pressure up by 1cm and not bothering with turning EPR on just yet. While most people like it, there are some people who don't.

Thanks so far I have just upped the pressure to 15, 16 hurt my ears (eustachian tube dysfunction). So far, no complaints from my wife, so I will check the stats again after a couple of days and see how it looks.

Thanks for all the help
