Should I up my pressure?

Post by bkkjames » Wed Jul 31, 2013 11:00 am

My doctor prescribed an APAP with a seting of 5-9. However, this was an educated guess since there was no titration done during my sleep study. My study showed an AHI of 31, with the vast majority of events being hypopneas rather than apneas. I am travelling without a PC so I cannot get full data from my S9. The sleep report is showing a pressure of 8.4 over the last week (I believe that means 95% of the time the pressure is below 8.4). The AHI fluctuates but last night it was 2.4.

I was wondering whether I should try upping my pressure to, say, 7-11, or whether I should wait for a week or so until I can get home and see more data from the machine.

BTW, if someody could PM me info about how to get the ResScan software, that would be great.

I don't know that I would change the pressure until you can see the reports.
Based on AHI..not needed.
Though that minimum of 5 cm may seem like not enough air is moving and if that is the cause a little increase to 6 cm won't hurt anything.
That 95% maximum number doesn't really point to needing more than 9 cm and until you can evaluate the AHI event category breakdown...as in how much of it is maybe centrals and wouldn't be responding to pressures anyway...so unless you were seeing 9 cm maxed out for prolonged periods of time accompanied by Obstructive Apneas and Hyponeas at the same time frame...there's no real urgent need to change it.

I will send you a PM in just a minute with ResScan information.
You can also use SleepyHead...links in my signature..if you didn't know that already.

Are you using EPR? If so at what setting?

So a little more pressure probably wouldn't hurt anything but I don't see it being critically needed.
You really need to see the software reports and the event category breakdown and the pressure line to see when and what is going on.
When making adjustments to pressures using APAP mode...it's the minimum pressure that carries the most weight and is most important. Have it optimal and the maximum often stabilizes and doesn't go as high or as often.

I am not using EPR - the machine came with it turned off - but I had been wondering whether I should turn it on.

During my brief experience with a nasal mask in the sleep lab, I found it very difficult to exhale, but I was pleasantly surprised not to have this problem at home. Not sure whether it's the S9 or the low pressure that's making the difference. The feeling I had in the sleep lab is similar to what happens when I do a mask fit test on the S9.
