Does anyone know where to find a guide to how often United Healthcare will pay for replacement CPAP equipment?

It actuallly depends on your plan. There are several different types of plans available--We have choice plus but there are others and it doesn't necessarily depend on the medicare guidelines unless yours is a medicare type plan. My DME told me i could only have a mask every 3 months, however when the dr. ordered another type because of sores on my face, I called the insurance company and asked and they told me there was no limit on my plan other than I couldn't go over the yearly limit of $2500 for durable med. goods. I would call your information number on your card and talk to them.

United healthcare has nothing online or by phone.tried several times to call never got anyone that could tell me when they replace anything.Even asked them "so I am paying for insurance but you can't tell me what it covers" and they reply with your dme will submit it and then you will know.UHC doesn't list sleep doctors or sleep labs on their site either its like a black hole in the universe. On my plan they pay 100% of the sleep study so maybe they dont wan't anyone to know about sleep apnea.
