Last night CPAP numbers

Post by d.green » Tue Dec 02, 2008 10:30 am

Here are my numbers from my CPAP.
Press: 8.2
Leak: 0.18 l/s
AHI: 10.8
AI: 1.2
HI: 9.6

Can anyone tell me what they mean.

Dan,
I've been looking at this image you posted, again, where it says "Respiratory Events":
Image
You were right - there are far more central apneas there than there were obstructives.

The problem with your sleep disordered breathing is that although you don't have many apneas/hypopneas (you quoted 7.5) the average destaturation for those is too low - 89 - and the minimum is bad.

If I were you, I would ask for an overnight pulse oxymetry at home, and see how the events reported by your machine correlate with your oxygen saturation.
I would also ask StillAnotherGuest, (know as SAG), a forum member who heads a sleep center, if he could help.

Base on the AHI it's reporting, it doesn't seem to me the machine is doing much for you - and I honestly don't think you have garden variety obstructive sleep apnea.

You need a good professional in your team.
O.

I find it somewhat strange that the bulk of the CA's started only after the Bipap therapy was initiated.

Also look at the pressure changes for that switch to bilevel. I don't know if that is standard or not to jump that much in the switch.

Do you have a copy of the diagnostic PSG to see events there?

Interesting point. The CPAP left off at or just above 10, and the Bipap IPAP starts off at or just under 15. I also don't know if that's protocol or not, but......
It's not like there were any obstructive events during the CPAP hours, which they were tyring to titrate out. What were they trying to titrate out between 23:00 and 03:30 anyway? The 3 (5?) lone Hypopnea's ocurring once an hour?
Snores? Those are not marked anywhere.

I think we have more unanswered questions right now than answered ones.

What lab? What are the credentials of the Tech? The Doc?

your PSG shows you are one of those that do worse on Bi-level therapy. Did/do you do a lot of tossing and turning during sleep? If so that can account for a lot of the CA's seen on your PSG. I don't see any "Mixed" apnea on that PSG so that is a good thing.

Based upon your CPAP numbers above,

Think I would try a .5 cm pressure increase to 8.7, with the CA's seen on your PSG you don't want to go above 9.0 cm pressure.

Leak looks good at 10.8 L/m
HI a little high but AI is what you want to look at. Get AI the lowest and that is about
all you can expect to get.

Note: When AI goes down, HI may go up at the same time, so you may have to fine tune in .2 cm increments to get HI to fall. Your numbers really don't look bad when you toss out the HI's.
