I appreciate all of the suggestions but I got the answer from Mako technologies. They e-mailed me and told me to change the "Smart Card Services" from manual to automatic... a quick easy fix.
NightHawkeye - When I insert the card into the reader, only the red light turns on. I don't see any green at all. When I am downloading data from the card, then the light changes to green.

My,My,My, It's a wonder these computers work at all. A person could sure cause havock changing a lot of things in services. I've made a couple of changes in there myself, but I never felt good about it. Jim
Encore Pro Version 1.8-Smart Card Reader Mako VS Activa Card

Hop,
My husband has the same problem you did and he is using Encore Pro ver 1.5. He has checked and rechecked everything but whenever he tries to download data he gets the message “smart card reader not detected”. The suggestion from Mako Technologies about the Smart Card Services did not solve the problem. The light on the reader turns green for about 40 to 60 seconds after the card is inserted, then turns red. He is using a USB port on the computer. The device manager says the reader is working properly. Any suggestions??

Sounds like it might be a problem with your smart card or software. I would recommend that you e-mail Mako Technologies. They responded really fast to me and were very helpful.

Are you sure you're using the correct driver?

If you haven't already, download this one:
DT3500-ISW-Encorepro_v1_3.exe
The system may say it's working correctly, but Encore Pro needs that particular one.

If you're indicating the 1.2 version, that's not the right one.
The correct version is NOT at that location.

I downloaded this one and it worked the same as the driver on the encore Pro cd.

If I understand you correctly, you're saying that you did not use the special "Respironics" driver to install your Smart Card reader. Instead, you were successful in getting it to work with the "manual install" driver available from Mako. Do I understand this correctly? (The rest of us here used the "special" Respironics driver.)
Suddenly, your LED sequencing being different makes sense. BTW, congratulations on being the first here to discover this.

OK. Yeah, it's the "2800.sys" definition.
I had downloaded those files a long time ago (just to have them), but never actually used them.....just used the one on the Encore Pro software CD.
I think it's probably a little easier (?) to use the installer (.exe).

OK, let me make this clear. I was having problems with detecting the card reader. I was using the driver that came on the Encore Pro disk. I was trying everything I could think of to solve the problem so I downloaded the above driver and loaded it. After the Mako Tech guy e-mailed me to change the smart card services to auto, the detection problem was solved and I still have the downloaded driver installed. (whew!)
I don't know, maybe I should reload the one off of the disk. But this one seems to be working ok. Actually, when I tried to load the downloaded driver, the device manager said it was no better than the one I already had. So I had to delete the original driver in order to load the new one.

The "2800.sys" is supposed to be the correct driver.
If it's working OK......I wouldn't mess with it if I were you.

But . . ., maybe some of the problem installations are a direct result of the "automatic" installer.
I'm gonna have to do some experimenting with the Mako driver in my virtual machine installations in Linux. I'm hoping this other driver is a little more forgiving than the "Respironics" driver which I've never gotten to work consistently in the VM's. The best I can get is consistent downloading in Virtual Box if I allow Encore Pro to automatically erase the card. (If I don't allow the automatic erasure then the Smart Card data download sometimes crashes.)
