You appear to be sleeping and breathing normally in that section. You want more of that. :D

In the section to the left, you are having events, breathing erratically and probably having some arousals. You want less of that. :(

so what does it mean when my "shark fins" turn upside down? stealth mode? dead shark?

It means that there were brief periods where your sleep wasn't terrible, and the machine started dropping the pressure, only to have it go back up because you had another event.

Should I be worried that pressure was pinned at 20 for so long?

Well, only if you want to get to a lower AHI... you need more pressure to control those obstructive events.

Many of use have been able to reduce our pressure requirements by wearing a cervical collar or similar anti-snoring collar. My pressure was hitting 18 to 20. Once I started wearing a collar, the pressure requirement are 12 - 14. A discussion of various collars is here - viewtopic/t172782/Success-with-Dr-Dakot ... evice.html

Firstly thanks to all for their advice and input.

Its been a couple months now on CPAP (again) and I would say this my AHI has fluctuated between high 2 and as high as mid 6. But I can say it feels largely random. So very hard to isolate all the variables in my sleep (though the 6 was after a night of drinks)

Even tried the Dakota neck torture device and it didn't seem to work and for whatever reason made my aerophagia go through the roof.

But my latest concern is that my machine settings are not right. for example, last night:

And closer look it seems the pressure doesnt adjust at all:

Appreciate any thoughts on settings or otherwise. My flex setting is on "2" - I did try putting it at 3 one night and the aerophagia was worse......

The events you zoomed in on...not real...post arousal events...semi awake or awake breathing irregularities getting flagged by mistake.
The machine won't respond unless other signs of obstruction are going on like flow limitations or snores or real OAs/hyponeas within a certain period of time.

Even if that CA had been real...the machine won't/can't do anything about centrals.
Even if those OAs had been real the machine doesn't increase the pressure at all during the OA/hyponea but instead it waits and re evaluates things to see how it could better prevent those apneas from happening again. The machine absolutely will NOT increase the pressure during an event. It simply doesn't work that way.

As pugsy said that looks to me post arousal roll over or something. But your machine did respond you need to remember an APAP does not increase pressure to treat an event but increases pressure to prevent the next event happpening. You can see from your zoomed graph that the machine increased pressure shortly after the Second OA as the pressure line rises at about 01:16:00 ish, Your machine will do nothing to treat a CA.
