I would try to get the script from the doctor - not sure Med Records can give it to you, or will make you sign something for it.

Already got it! According to the law, you can get anything that you ask for from your chart. Our clinic has 2 ways to do it... You come in and ask, and then they put you in line for the nurse, and then the nurse comes out eventually and takes you to an exam room and you go through your chart and then she'll make copies of anything you want. Or if you know exactly what you want you can just call the records office and they will print it off and put it in an envelop and then you just ask for it at the clinic front desk. If you want something that's a lot of work (many pages of copies/printouts) they have the right to charge you a reasonable charge, but I've never needed that much.

I know that some people have to get nasty to get their records, but so far every office I have ever dealt with has been great!
