Do I have really weird flow curves?

Post by justinjustin » Fri Oct 10, 2014 8:27 pm

I'm on ASV Standard EPAP 6, IPAP 6, maxIPAP 12.
As I'm looking over my data, on many/most nights I'll see long stretches of nice rounded and regular curves like this.

But then I'll see some 5-10, or 5-20 minute unstable stretches like this.
Does anyone have an idea as to whether this is normal or what's causing this?

And then putting it into CPAP mode at 6cm, I'll have 'cut off' inspiratory peaks.
I assume supports me having UARS??

Justin, I have a lot of stretches that look like yours. Irregular:

Flattish:

I can't answer your question about UARS but my neurologist thinks that I probably have UARS based on history and a visual ("open your mouth") exam. (I haven't had an attended sleep study yet.)

Conventional wisdom is to not worry much about anything other than gaps in the breathing. Flat topped waveforms may indicate flow limitations, but that's not always a big deal either.

For myself, I have observed a lot of the cutoff flow rate thing. I suspect it's the epiglottis partially blocking the airway when the muscles are relaxed.

I wonder then what something like this indicates....

Those breaths with low latency to inhalation might indicate microarousal due to airway resistance.

Why keep the EPAP at 6.2? It doesn't seem enough. I never used an ASV. Do you feel you get good sleep with this pressure setting?

Anything higher and I start to get aerophagia. I've had EPAP up to 8cm, but no difference; whatever my settings, I feel awful on the ASV.

A year ago I couldn't imagine I can tolerate my current pressure 17/14. I am trying 20/17 tonight (I am surprised that I need such high pressure since I am only 30 and weigh ~180 lb).

Here's my results again with 14/6. This doesn't look healthy, or maybe it's normal?
