Bi-level to Apap?

I have COPD and the hardest number to adjust for me was the Epap or exhale number. When you have COPD it is hard to exhale as it is and when you have to exhale against a high pressure sleep is very hard. I don't even know what ps pressure setting means but I know mine is 6 and it seems to work fine

PS is Pressure support...the difference between inhale and exhale.
EPR pretty much is Pressure Support called EPR..difference between inhale and exhale.
Only thing is..with the APAP machines in the ResMed line....EPR can't ever be more than 3...it would feel like using your machine now with PS of 3. I don't think you would like it given your history. I thought I remembered your having some COPD issues..and that's why I mentioned it.
If you change your PS to 3...that's what EPR at 3 will feel like. If you didn't like 4 or 5 PS...you sure won't like 3.

Because people with COPD have trouble exhaling anyway...exhaling against cpap therapy becomes more of a challenge.
With COPD the lungs have to work harder exhale and empty the lungs.
PS of 3 isn't as easy to exhale against as PS of 6. The greater the difference between inhale and exhale the easier it is to exhale.
Hmmm, I may need to take a closer look at my dad's Respironics. I'm not sure he's on a bilevel machine (system one w/C-flex?) But I know his COPD is stage three and he's had a great deal of difficulty this last month or two. I don't think the nursing staff at his facility have a clue about XPAP therapy; it took nearly three days to get his personal machine "approved" for use in their building...

If it says "CFlex on the machine" it's straight cpap and not a bilevel device...the bilevel Respironics are called BiPap and the exhale relief is called BiFlex and is clearly stated on the machine.

CFlex at 3 would offer the greatest possible reduction during exhale and even that is based on rather forceful breathing which is not something someone with bad COPD can normally do routinely. It's not 3 cm though...at the most under forceful breathing maybe 2 cm reduction. Respironics exhale relief doesn't work like ResMed's EPR. It base on air flow (breathing).

Bipap would probably be much more comfortable for him...and in this situation even EPR at 3 might feel better to him.
