Mask type for bipap therapy

Post by dtsm » Tue Aug 05, 2014 8:54 pm

Friend in singapore asked for help. His dad now in hp and requires bipap therapy. Doc recommend respironics model. Interesting is also instructed that patient using bipap device requires full face mask? Is this true...nasal or nasal pillow won't work?

As long as you have been around here, you should know Full Face Mask, allows mouthbreathers to get good treatment, where a nasal mask may allow the treatment pressure to come out of hour mouth, making your treatment poor or void. Jim

BiPap users aren't limited to full face masks.
Lots of nasal cushion mask users as well as nasal pillow mask users are quite successful with their Bilevel machines.
I used nasal pillows with my PR S1 BiPap and now with my S9 Adapt ASV bilevel machine.
I think a lot of it might be the idea in the past that the other masks don't do well at higher pressures but that's not true.
If someone can keep their mouth shut for the most part...they can use any mask they want.

For as long as you been around, you should know:

1. Full face masks are difficult to fit properly, more susceptible to leakages, least popular of mask types.
2. Cpap and bipap therapy differ. I know nothing about bipap, thus the query.

And your answer was absolutely useless BS! Go away....

I started with FF masks, I use both types successfully, but when mouthleaking gets too high, 40 lpm, for me it's back to the FF, so treatment works. My pressure is 14.5 cm. AHI under 2.0! But you have been here a long time so you may be his expert. Jim

Nasal masks are the best choice if the patient can use them without excessive mouth leaks.
There are successful strategies that are discussed on this board on coping with mouth leaks with nasal masks including:
Chin straps, self training to not mouth breathe, partial taping of the mouth, cervical collars to support against jaw drop while asleep, etc.

If mouth breathing is a serious issue, then either a FFM or an oral mask will be better.
FFM's do have more problems with leaks than either nasal masks or oral masks because of the increased surface area needing to be sealed.

Range of pressure should not be a factor in which mask type is best for a particular patient given current mask technologies.
