FFM mandatory on ASV?
Hi,
I'm using ASV and was told by Krakow's team that FFM is important to use on ASV because it can better detect events. True?
I had mouth breathing issues on CPAP with nasal mask, but maybe I'd do better with an ASV on my nasal mask, at a PSmax of 14?
I'm considering a switch back to nasal on the ASV, because my FFM is causing tons of wakeups from leaks and squeals. Anyone else nasal on ASV?

Re: FFM mandatory on ASV?
I am using a S9 Adapt SV machine and a nasal pillow mask.
My settings are
Min EPAP 6.4
PS...3 to max of 15
IPAP max...25
I see IPAP up to 22 fairly often but mostly around 19.
FFM used to be mandatory with the older technology ASV machines but not so now.
Try your nasal mask to see what happens.

out of curiosity, what *kind* of asv do you have? put it in the 'additional comments' on your equipment page. a vpap adpat 36037 maybe?justinjustin wrote:Hi,

I use Resmed S9 VPAP Adapt 36037. (not sure why my sig doesn't display it. It's in my profile. Anywhoo..)

Re: FFM mandatory on ASV?
Nope. I use an oral mask, Oracle 2 and mostly have zero leaks. My IPAP routinely goes to 25 cm H2O for long periods (2 hours or more). It did take me a while to get leaks under control but that is common with all mask types at higher pressures.

the folks that provide the board don't sell it, so it doesn't show up.justinjustin wrote:palerider wrote: out of curiosity, what *kind* of asv do you have? put it in the 'additional comments' on your equipment page. a vpap adpat 36037 maybe?

the only way to get it to display is to put it in the comments

Re: FFM mandatory on ASV?
I use the Wisp nasal mask with my DS960TS ASV. I also see IPAP above 24, and above 20 cm for hours at a time.
I did find a chin strap helped to curtail mouth leaks and brought AHI from 6.9 to under 1.0 many nights. When my AHI does get close to 3 cm events are usually clustered in a one or two hour period, possibly around an arousal.

Who is "Krakow?"
I presume they're concerned the higher resistance to airflow in a nasal mask can make central detection harder. I wonder if the more recent machines do a better job of this than older machines. I would expect that a nasal pillow mask would have more of a problem of this than a nasal mask.

Re: FFM mandatory on ASV?
I used both the Swift FX nasal pillows and the Wisp nasal mask for a few months when I got my ASV machine last spring. They generally handled the higher therapy pressures and the pressure variations nicely, especially in the leak department. My problem with nasal systems was due to larynx and tongue architecture which allowed some blow-back from my pressurized airway... this resulted in some weird and uncomfortable warbling in my larynx which tended to wake me up. That was just my anatomy tho - most people should have no problem using nasal pillows or a nasal mask, provided they don't mouth breath.

I like my ResMed AirFit F10 FFM - reasonably low leaks for my ASV therapy. I'm currently using a PR S1 AutoSV 960P Advanced. I also keep a ResMed S9 Adapt as backup. I use a heated Hibernite hose. Still rockin' with Win 7 by using GWX to stop Win 10.
