Help dealing with DME

Post by urbanscribe » Sat Jun 28, 2014 6:04 pm

As expected When the DME came they have me the S9 escape.

I am familiar with the autoset and know perfectly well I won't get any useful analysable data from the escape only compliance on the sd card.

The insurance company denied a titration study and my GP does not have a sleep background. The DME sees an open 4-20 pressure range as the end point. I need the data to evaluate the progress of therapy and understand the right pressure.

What are arguments I can make to the DME for them to swap the escape for an auto set? What has worked for you?

The first step is to get your MD to write the prescriptioms for exactly what you want. Then write dispense exactly as written.
When the DME delivers the wrong machine refuse it. Now it is more dificult since you accepted the machine and signed for it.
Call the DME tell him you can not accept the escape and insist that he replace it with an Autoset. Be as firm as you can. Check with your insurance to verify which DMEs are in network and threaten (promise) to switch. Then do whatever is neccesary to get what you want.

escape, or escape auto?

one argument is that the escape auto has "apap" mode, whereas the autoset has 'autoset' mode.

apap mode is similar, if not the same, as the s8 auto, and doesn't provide detection of centrals, and won't go above 10cm pressure if needed because it can't tell what a central is and what isn't. it's a vastly inferior program.

Tell then that you borrowed an oximeter and are noticing that you are still experiencing desaturations below 90%. Also tell them that the machine is waking you up with mask leaks and you want to track those leaks to see when and how often they happen. Tell them that after waking up from the leaks it seems that the machine is chasing the leak and doesn't back down in pressure until you shut it off and turn it back on again.

If none of that works you can find another DME or pick up a slightly used machine for a price that may be close to your co pay.

Thanks for asking
Escape Auto is what they have me
I believe from reading carefully here it does not record detailed data like the autoset

correct, the escape series do have, nor record, detailed data. simply usage and if you're lucky, ahi.

refuse it, demand the autoset.
