APAP 'trial' and pressure levels...

ok, so 7 hours with the mask on, but the first 26 minutes (before the big break) I didn't sleep plus the last few minutes I apparently has a large leak (maybe why I woke up? lots of hypopneas...1.52 vs 1.52 on the large leak) very low obstructive and clear apneas though. Now, looking at the pressure, which I have back on APAP, min 10, max 13, does that look ok? or should I increase min to 11? (I'll have to go back and look, but I think whatever I set the low to, it seems to bounce up one or two psi's.) or should I drop it lower? Not really clear on how all the APAP works. I 'lost' my APAP for a few days, until I changd the APAP TRIAL time. apparently 30 days is all it'll do, is there a way I can get rid of the 'trial' part of it and make it permanent APAP?

Not permanently...not with the 460 machine. It offers 30 days max of APAP mode trial. There is a way to reset the machine to get another 30 days...and then another 30 days but I think the limit is 5 resets.
This reset involves erasing ALL past therapy history though. If DME or insurance compliance is still need obviously not something a person wants to do.
What is done is just a reset to zero all past therapy data...it's a choice in the clinical setup menu under the "Info" section.
Then the machine thinks it is brand new again and the 30 day apap trial starts all over again but it isn't limitless. Eventually the reset won't give you any more APAP capability days.

Your only large leak time occurred at the end of the night..may have woke you up or you may have already been awake.
1.52...that means for 1.52% of the night you were in large leak. It's easily seen and nothing to worry about.
Large leak territory is somewhere between 80 and 90 L/min for your machine with your mask and your pressure....you never come close the other parts of the night.

that would be an hourly average and well within acceptable limits and NOT anything that needs more pressure to resolve from a technical stand point. Not considered "lots" We are normally happy if the entire AHI is less than 5.0 and yours sure qualifies.

No need to increase minimum to 11 cm at all IMHO. The spiky sawtooth pattern is normal way for the APAP mode to test things. As you can tell the only time it wanted to go up and stay up was around 7:30. Unknown what was going on at that time but most likely supine sleeping or REM stage sleep. It was brief though.
If anything you could probably reduce the minimum from a technical aspect but if you are comfortable at this pressure...no need to change. Eventually with this machine you will end up having to use a fixed cpap mode pressure though so it might be useful to see how low you can have the minimum and still have the machine do a nice job preventing the airway collapse unless you are happy with fixed 10 cm which would probably do a great job.
