MMA/GA Medical Insurance

I am a 26 year old male who has been suffering from OSA for at least 8 years (diagnosed in 2002). I have been through the UPP surgery and have not been had any success with CPAP.

I'm currently looking into the MMA/GA option. I have reviewed many of the previous postings on the issue, but never saw anything about covering the costs of this procedure. Is this something typically covered by medical insurance or is it considered cosmetic in nature? Also, what is the average cost the procedure if it is done out of pocket?

I'm at a place in my life where I have significant time to recover from the extensive procedure (lots of sick leave), but it would be really great if insurance defrays some of the financial cost.
