Can anyone tell me where the best (cheapest) place is to buy the software and reader (my insurance won't cover)? Is it true that you have to use a proprietary card reader?

The least expensive I know of is cpap.com for the software:

Just make sure you're using Internet Explorer as your web browser, other wise you will have problems placing your order.
Total cost a little over $130.
