The doctor who did my original testing is now out of network. My family doctor is willing to write me a new prescription, and has my records. What info do I need to have included?
In the past, my Rx was sent directly to the provider (Apria) who never bothered retaining a copy. Until I read this forum, I didn't think about having the prescription given directly to me.

You can still request your original prescription. It's part of your Medical record and you own it--make the request in writing (fax is OK) and they are required to provide it to you.
As long as you meet the testing criteria for OSA treatment under Medicare (should already have been established by your former doctor), you don't have to worry about Medicare. You have to worry about the DME. They will try to give you the cheapest equipment possible, to make maximum profits off you. So your RX can be very specific so that they have NO wiggle room to foist a brick off on you.
Something along the lines of:
"Auto-Adjusting CPAP machine with heated humidifier and patient access to AHI and leak information. Mask of patient's choice." Or, if you know exactly what machine you want, ask the doctor to WRITE it in the script, i.e.
"Philips Respironics Sytem One Auto Adjusting CPAP with heated humidifier and mask of patient's choice."
"Patient access to AHI and leak data" is important because they can't foist a ResMed Escape on you which has AHI data but NO leak data.
