What's going on????

Post by Janknitz » Fri May 14, 2010 1:32 pm

The last couple of nights, I've been feeling like my OSA is "breaking through" the CPAP therapy. I don't have software, but my screen numbers look OK--AHI's remain below 1.0 and my 90% pressures are between 11 and 12. My nasal pillows have been a tad more leaky, but not significantly so.

What's happening is that I'm having those OSA symptoms again. I wake in the middle of the night in a puddle of sweat and my mind running a mile a minute full of anxious thoughts. I'm pretty sure that this was happening regularly before because of a deep apnic event during REM sleep, and that seems to be the case here--at about the same time of night, too (around 4 a.m.). When I wake the pressure is pretty high (can't see how high in the dark even with a flashlight because of the lousy PR screen!). Sometimes I hit the ramp, though not always--sometimes I can fall back to sleep without moving around too much.

Last night I just couldn't get back to sleep (again, that was a common scenario before cpap). I had to get up and change to a cooler and drier nightgown and plug myself into the iPod for a while before I could go back to sleep.

It's a little frustrating without software to be able to see what's happening. I am not mouth breathing to my knowledge, not sure what else it could be.

Any chance you can get your hands on the SW? Mouth leaking *could* be a problem, since for many of us it only starts once we get used to the therapy and start getting good sleep. You might want to change the pillows, it's amazing how even just a little bit of a leak can affect some of us. One last suggestion I can think of, might want to try straight CPAP to see how your AHI reacts and whether that might be causing some unwanted arousals... and/or try bumping up the minimum pressure, maybe one or two long A's are still getting by and that's causing a wake-up.

Sorry the honeymoon period's over , but I'm sure you can get back to good sleep!

Hmm, can't afford the software right now, but maybe I'll call the sleep lab and see if I can bring my card in so we can take a look.

Sounds like a good idea jan. just apply some good old fashioned troubleshooting to the issue!

Get them to give you the pdfs of the data (of all the days that are recorded). after at all is YOUR therapy right?

just another thought, while most people don't have problems with central apnea's, if you're noticing that your pressure is very high, it could be an issue. lowering the upper range might help. on the other hand the pressure might be higher to compensate for leaks (mouth or mask). hopefully your data will point to the real issue.
