Central SleepApnea/Need help with ventilator settings

I have a ResMed VPAP Adapt SV.I now have a Rx of 25/10/5 for a Respironics BiPAP auto-sv. I contacted both companies for conversion iinformation and came up dry. Does anyone have any suggestions with this Rx how the ResMed should be set? Thanks.

Greetings Stan!

First, you have my empathy about dealing with Central Sleep Apnea. I also fight with it.

Now some house keeping ... I see that you have registered. You should also list your equipment in your profile. Why? Well, I'm going to ask some questions that the equipment list would have instantly answered. Don't know how to do that, here's a pointer:

The VPAP Adapt SV uses the following values:
  • EEP (or end expiratory pressure) in cm H2O
  • Min PS (or minimum pressure support) in cm H2O
  • Max PS (or maximum pressure support) in cm H2O
Based on your prescription, the VPAP Adapt SV was set to EEP of 10 with a Min PS of 5 and a Max PS of 15 ... but I don't know. You need to provide more detail so I can help further. (That would make the maximum pressure 25, the minimum pressure 10, and an inhalation pressure that is between 5 and 15 centimeters higher than the minimum pressure).

Anyway, the BiPAP AutoSV uses the following values:
  • EPAP - Exhalation Pressure in cm H2O
  • iPAP Min - Minimum Inhalation Pressure
  • IPAP Max - Maximum Inhalation Pressure
To convert from your VPAP Adapt SV, you would set the EPAP to 10. You would set the IPAP Min to 15. And you would set the IPAP Max to 25.

Finally, here's the question part:
  • Can you provide more detail from your prescription?
  • Or ask your DME?
  • Or read the settings off your old unit?
  • Do you have a BiPAP AutoSV Advanced or just a BiPAP AutoSV unit?
  • Does that conversion make sense?

Can you confirm that your Vpap Adapt SV does these things ?

- allows Min PS to be set below 3 CMs
- allows Max PS to be set

Reason I ask is that my machine has a default Min PS=3 that can't be reduced & also
there is no setting to adjust MaxPS, that defaults to 20 on my machine.

I know an Enhanced Vpap Adapt SV came out but I can't find a clin manual that indicates
if the settings were any different to the earlier model. I am thinking they must be as
I am sure you would have got the info you posted from your own machine.

Can you explain what the 25 10 & 5 represent ?
and can you advise what Mode the Dr intended the Bipap Auto SV to be in ?

Those settings mean many things depending on which Model Bipap Auto SV they apply to. The intended
mode also helps.

John - my questions answered

OK I located the 'Advanced' Clinitians manual & on page 13 it mentions min & max PS as follows ...

The default settings for maximum (MAX PS) and minimum (MIN PS) pressure
support will suit most patients. However, the clinician can adjust the upper limits
for MAX PS and MIN PS in the Advanced menu.
Note: The VPAP Adapt SV will automatically limit to ensure that:
a. MAX PS minus MIN PS is greater than or equal to 5 at all times.
b. EEP + MAX PS will not exceed 25 cm H2O.
To satisfy these limits:
a. Setting EEP automatically adjusts MIN PS then MAX PS.
b. Setting MIN PS automatically adjusts MAX PS.

Also it shows that 3 is the Minimum MinPS setting :-
MinPS can go up from the default 3 CMs
and
MaxPS as default 15 and can only come down from 15 CMs.

Yes that would have been fun - I now have so many bundles of docs that I am losing track of them & can't find them as easily as before.
I actually went to Resmed for the one I eventually looked up. I probably have 5 different copies salted around the place (web sites).

One day soon I'll do a big re-org & try to create some structure suchas

By brand
By type of machine (cpap, autocpap, bilevel, asv)
By type of doc (users manuals, clin manuals, patent docs, service manuals, presentations etc:)



What I really wanted to find out was if the 3 CMs minPS was still applicable. It says a lot about how Resmed view the Vpap Adapt SV
when comparing the Bipap Auto SVs ability to do Cpap with pressure support. It is an interesting difference.
