25yo w/ Severe OSA Considering Surgery

Welcome. You are in good hands here and seem to be very involved in getting therapy, so you're half way there. I was going to ask about your leak rate, but then I read Pugsy's post. Is it possible that your therapy isn't as effective because of leaks? Once you have reached a point of significant leaks, your CPAP therapy is pretty much doing nothing at that point. Let us know what your data shows.
