My plan BC-BS allows for replacement masks every three months. My original DME, the one I bought the CPAP machine from, was out of plan. The difference is: in-plan only charges me 15%, out of plan charges me 30% plus the hefty coinsurance fee.
Now my question is: what will I need to provide the new, in-plan DME in order to get replacement parts? Do I have to go back to the doctor and get a new prescription?

You have to ask the DME what they want.
It might be they will do the legwork and get the information, or they might be able to get the other DME to fax it, but it might be that you have to get all the paperwork yourself.
I don't think there is any one fixed rule on this.

I agree and in fact take that as a "test" of new DME's. Not that I have a lot of experience, but when I did switch, the new DME went out of their way to make it easy for me.
