Titrated too low ?

Post by luke » Mon Nov 10, 2008 1:42 pm

I think I was titrated too low at 7 from the sleep lab. In about a week of using my auto unit (set for 7-20 at first) I noticed that the LCD screen
numbers would consistently give a 90% pressure of around 10.5 give or take .2 or so. I do'nt have software yet but am planning
on getting it in the next couple weeks along with a card reader so I have to do with what I have for now.
I started bumping up the minimum number by 1 for the first 3 nights from 7 to 8, then to 9 then 9.5.
The whole time I noticed that the AHI went from 4.1 down to 2.7 in connection with raising the minimum number.
last night I had it set from 10-12 but havent got the data from the LCD screen yet. My leaks were almost exactly like what the
Comfort-gel brochure says they should be (around 28 or so) I did manage to get the leakage down to 24 by putting a little tape on the
swively thing in front. Not the rotating part but a seam which was puffing a little air.
I know that the LCD screen numbers may not be accurate but the AHI TREND was downward.
I think I will leave it at 10-12 for a couple weeks and get the card reader and software ASAP. By the way no mouthleaking because
I am taping from the very start with 3M micropore tape. That's my story for now.

Luke

Be careful with the mask, above the swivel is a piece of plastic looks like it makes two slots, air is supposed to come out of that, it's the vent for the mask. Due to poor manufacturing processes in CHINA, the vent rate varies mask to mask. They tried to figure a cheaper method of venting, but the only thing they could come up with that was cheaper, they ended up with nothing to hook the hose to. Jim

24 lpm is too low for that mask, borderline too low for any mask. I'd say you are blocking the vent. I know this because I intentually did this when experimenting with mask flow. 24 lpm is getting close to trouble. Too low a leak rate and you rebreath your own CO2.

Thanks for bringing that to my attention Jim. I took the tape off. I didnt realize that a mask NEEDS a certain amount
of leakage to function well. BTW I just also realized that I was using the lab version of the comfort-gel mask.
I think it's meant for single use only...at least it says so on the mask "single-use only" lab-gel mask. Its probably
built even cheesier than the "real" one ! Its part number 1025262 (the sleep-lab gave it to me)

I just got off the phone with a supplier for a card reader and a REAL comfort-gel mask.
2nd-day air UPS..

thanks again.......Luke

P.S I got your PM

I think (could be wrong) that the "single use" designation on the mask just means to use it only for one person...to not try to sterilize it for use later on someone else. Just my guess. Nonetheless, it's good you're getting a replacement mask. Keep the old one for "backup."

Good luck, Luke. Jim's a good person.

I think the FDA regulations are also different if a mask is designated for single use or multiple use. As you suggest, I think it also has to do with the way the mask can be disinfected for use by another patient. They probably have to do additional testing for both mask sanitation and mask lifetime for the multiple use disinfection methods. I can't imagine that they've come up with a disposable mask that you can just replace every day It took them years to get daily disposable contacts on the market, I think it'll take them a while to come up with daily masks (but oh think of the extra $$$ they could make from that )
