I HAVE A REMSTAR/PLUS AND I HAVE BEGGED THE PEOPLE FOR THE HARD WHITE FILTER IT USES FOR 3 MONTHS OR MORE. THE MACHINE HAS OVER 1000 HOURS ON IT. THE MEDICAL SUPPLY PEOPLE THAT GOT ME THE MACHINE ALMOST A YEAR AGO SAID THEY DIDNT CARE THAT I NEEDED A FILTER OT THAT I COULDNT SLEEP WELL OR AT ALL.....THEY WERE NOT YET PAID FOR THE 9MONTHS X $245.00..OK SO 30 OR SO DAYS PASSED, I CALLED AGAIN AND ASKED IF I COULD GET THE FILTER AND ASKED IF THEY WERE PAID YET...SHE SAID SHE WAS PAID. i SAID GOOD, AND NOW MAY i HAVE A NEW FILTER? SHE SAID THAT SHE WANTED TO SET ME UP WITH A NEW MASK...i SAID OK..i ALSO MENTIONED MY TRAY FOR MY HEATED HUMIDIFIER WAS A BIT WORN...SHE SAID i NEEDED A WHOLE NEW HUMIDIFIER....AND HAD TO MEET WITH HER TECH AT MY HOME FOR THE INSTALLATION OF THE TRAY THAT i REMOVE MYSELF ALL THE TIME...OVER $500 SHE WANTS TO BILL MY INSURANCE FOR A $5.00 FILTER...I SAID THAT ALL I WANT IS THE FILTER AND PERHAPS THE MASK...SHE SAID ALL OR NOTHING...I CANT TYPE WHAT I SAID TO THEM....AS A MATTER OF PRINCIPLE I REFUSE TO LET THEM ROB MY INSURANCE COMPANY.

I'd also let your insurance company know. Document everything and give the insurance company detailed instructions. Then find another DME.
Several years ago I went to a doctor who happened to be my company's recommended physician. My visit was simply to get a refill on allergy medicine that I had been taking for a couple of years. When they took me back to the office the nurse took my pulse and temp. I questioned why she had to do this for a simple prescription refill. I must have caught her at an "honest" moment because she looked me straight in the eye and told me that by taking my pulse and temp that legally they could bill me for a complete physical. I asked her about the ethics of such a thing. Her response was, and I quote, "you have insurance, what do you care?". I walked out of the office without my prescription, went back to my job and told the benefits people, and called my insurance company. Let's just say that he was no longer the "recommended" physician for my company after that.
Doctors and insurance companies like to blame lawsuits for our rising insurance costs. I've always wondered how much the "physical scam" costs us every year because I've never been to a doctor's office without them wanting the temp and pulse reading.

Think this post is a bit misleading, you said your humidifier was worn and they are replacing the whole humidifier, and they asked about a new mask which after 6 months you might need. So they are not billing 500 for a filter, t hey are billing 500 for a mask, a humidifier and a filter. Humidifiers are $150ish, masks are $80-$150 depending, and the filters are a few bucks, plus their markup. Yes they are charging more for the items, but it is not fair to say they are charging $500 for a filter when you asked about getting your humidifer fixed and you are probably due for a new mask.
