Need Some Analysis

Post by ejdischer » Mon Feb 19, 2007 6:25 pm

Downloaded info from my reader for the first two nights I've used my Soyala FF.

First night: AHI was < 1.
Last night: AHI was just under 5.

My leak max was around 120 for both nights while the average was around 40. I know the M-Series doesn't take into consideration the fact that the mask has an exhaust, so that factors into the leak rate. I was also a bit congested. I just found it weird that the first night with the mask, I had the best night I've had on APAP, and last night I had the worst in terms of time spent in apnea. I didn't have a large leak last night. But my 90% pressure was at 20 whereas the first night it was at 16. Any thoughts?

Could you post the reports? There may be a pattern happening that won't be apparent with just the numbers.

yeah your leak rate is out of control, machine STOPS responding at anything over 75L/m.

Pressure was 16cm one night and 20cm the next?

With OSA the pressure you need to stent your airway open does NOT change by 4cm over one night (mine hasn't changed in over 5 years).

Something is causing the machine to runaway with pressure, a large leak like 120L/m is the most likely cause.
You need to look in the Soyala manual on the flow chart and find your 90% pressure.

I'm assuming 16cm pressure is your last 90% pressure, on the chart for the FF, your leak rate should ONLY be between 35-40L/m, not 120L/m.

The M series will display actual leak rate, you must know the leak rate of the mask you are using to compare to the leak data shown on the LCD.

So if the M series says your leak rate is 120L/m (considered a Large Leak and impedes the ability of the machine to read and respond to events).

you take 120 minus the mask flow chart leak rate and come up with total External leak (external leak is the amount leaking past the cushion and your face). You are leaking at 85L/m, you must stop that leak.

Based upon your leak rate, all your data is invalid. You will need to adjust your mask to reduce that leak.

Your leak rate shown on the LCD should be between 35L/m and 40L/m from 16cm pressure to 20cm pressure.

Pressure was 16cm one night and 20cm the next?

Like I said your pressure requirement doesn't change that much from one night to the next. If you know what your 90% pressure has been in the past using other masks, I suggest you limit the Auto:Max pressure setting from the current 20cm. Leak or no leak, something is being misread to cause the pressure to "runaway" when you get up in the 16cm range it can very well be pressure induced centrals. Since your AHI went up along with pressure, that is my guess without seeing your diagnostic PSG. There is NO reason to leave your Maximum pressure at 20cm.

My suggestion:

Set Auto:Max=16 (or 17cm maximum).
Set Auto:Min=xx to anything you want.

To control your Large Mask leak set

Set Mask Alert Feature=On This will sound an alarm during the night (it is not that loud) to alert you when mask leak exceeds 75L/m.

Set Auto:Off = OFF Make sure this is set to OFF, when you have one of those Large Leaks, with this feature set to ON, the machine can turn off in the middle of the night, only when it senses you breathing will it restart (Auto:On is always on).
