Still exhausted on CPAP, please help

Post by gustavomfb » Mon Oct 19, 2026 1:20 pm

Hi everyone,

I'm 24 and have been using CPAP for about 6 weeks, but I still wake up exhausted and never feel refreshed, even after 9–10 hours of use.

I was originally diagnosed with moderate OSA (AHI 26). Since then, I've undergone MMA (10 mm advancement), tonsillectomy (palatine and lingual), adenoidectomy, septoplasty, turbinate reduction and pharyngoplasty.

I'm currently using a ResMed AirSense 10 AutoSet with a Resmed P30i mask, pressure 8–10 cmH2O.

My main issues are:

- **Mouth leaks:** My jaw drops during sleep, causing significant leaks and dry mouth. Mouth taping and a regular chin strap haven't solved it.
- **Residual events:** With EPR 3, I seem to have more clear-airway events. With EPR off, fewer central events are flagged, but hypopneas increase. I'm unsure whether these are genuine central apneas or arousal-related events.
- **Unrefreshing sleep:** Even when my AHI is around 2, I still feel exhausted. Previous sleep studies also showed very little REM and deep sleep.

My DISE showed V1 (AP), O0, T2 (AP), E1 (AP).

I've attached OSCAR screenshots comparing EPR on and off, including zoomed-in breathing patterns.

Here's my SleepHQ data from September (different settings):

I'd really appreciate your thoughts on whether my pressure/EPR settings need adjusting, what might explain the event patterns, and whether switching to a full-face mask would help with the mouth leaks.

Thanks for any advice!

Do you take any medications of any kind (even OTC) and if you do what is the name and dosage?

Do you remember waking often during the night or would you say you sleep soundly for the most part?

Any other physical or mental health issues going on?

Are you having any nasal congestion at night?

welcome to the zoo. pay close attention to pugsy and you'll get thru.

I quoted the one thing I may be able to help you with. to keep your jaw up, you need a firm cervical collar. This is the one I use:

I realize it's spendy. if I were you, I'd check your local grocery or drug store. look in the section where they have crutches or bedpans or suck. they'll be cheaper to see if it actually works for you.

good luck!

Being not wealthy like Moneybags Zonker, I use this one -https://www.amazon.com/Universal-Cervic ... ref=sr_1_9
