BiPAP Settings Help

Post by cbballantyne » Tue Oct 13, 2026 4:52 pm

Hi All,

I am now 2 years+ into my sleep apnea journey and am hoping to get some advice on next steps for my BiPAP settings.

I actually posted here around 2 years ago and was given some initial advice, since then I have been down the rabbit hole trying to dial in my settings. The simplified version is something like this:

1. Was feeling cognitively slow, struggling with memory and energy for the years leading up to my sleep study, sleep study revealed mild sleep apnea based on an AHI of 6.1, but during REM it jumped to 18.
2. Started out with CPAP (Resmed Autoset 10), went through all the usual trials and tribulations of different masks, mouth taping, dialing in pressure etc, and although I saw some improvements, still never felt back to "normal" or feeling good consistently
3. Reviewing my OSCAR results, I saw signs of what I think is UARS, and as a light sleeper, my thought is that although my AHI is low, I may still be having arousals that are unreported either due to UARs or the machine iteself. This brought me around to trying the resmed aircurve 10
4. With my first crack at settings for the Resmed Aircurve 10 BiPAP S, as an approximation to my CPAP settings (pressure of 11 and ESR 3), as well as increasing trigger and cycle sensitivity to high, I saw additional improvements, and Glasgow index was improved from ~1.5 to ~1.3
5. Seeing some improvement on BiPAP renewed my thoughts that I can actually see improvement if I get the right settings but I'm hoping with the wisdom available on this forum I can expedite further trials as after 2 years in I am losing steam doing this on my own.

Based on my Oscar data, I still see evidence of flow limitations leading to arousals, but I also seem to be experiencing aerophagia if I increase my pressure further. Further if I increase pressure support I see CA events creeping up.

The BiPAP has even more settings like rise time, trigger/cycle sensitivity, easy breath on/off, Ti min/max etc, so I'm hoping to get advice on what my next steps should be based on how my Oscar data looks.

Any help with this would be greatly appreciated!
