Resmed AirSense 10 AutoSet - Is automatic setup better than fixed pressure?

Hi guys! Do you know if the algorithm of CPAP Resmed AirSense 10 AutoSet is trustworthy? I mean, I have a prescription from my doctor with min and max pressure, but I already had to change this pressure some times because I'm still adapting as I'm using it for 20 days now. Some people say that it works very good, other say that in AutoSet (without setting pressures) it can use an excessive pressure that is unnecessary. I'm using that for 20 days and my reports say that min pressure I achieved was 8.9 and max was 10.7. Right now it os configured min pressure as 9 and max as 11. Should I keep oit set or should I trust on automatic set?

For most people most of the time the automatic algorithms are better than fixed pressure. There are exceptions, but for most people the automatic pressure settings work well. There are reasons to fine tune both the minimum and maximum pressure, but within a reasonable range the machine tries to use the minimum pressure that's necessary.

I suggest you get either OSCAR or SleepHQ. Once you have actual data its relatively straight forward to determine if its necessary to change minimum or maximum. There are some other settings as well, EPR and response for example. Get some data and there are people here who can help you interpret it.

I prefer auto adjusting pressures myself.
I learned a long time ago that my pressure needs during REM Stage sleep (where my OSA is much, much worse) were a LOT higher than what I would need during non REM sleep.
If I were to try the fixed pressure route then I might need 15 to 18 cm all night long....compare that to using maybe 8 or 9 the other parts of the night. Which would you want to do???
REM is maybe 20 % of the night.

The auto adjusting algorithm actually won't go higher unless it thinks it has a need to....now sometimes that reason isn't the greatest reason and when that happens fixed pressures might be a better option. Flow limitations come to my mind because sometimes nasal congestion can cause the machine to think it needs to go killing FLS with more pressure but nasal congestion FLs won't respond to more pressure. That's really about the only time that the machine will increase the pressure and get it wrong.

In general the machine's auto algorithm works quite well despite what you may have heard at other websites by people who don't think the same way.

A properly configured APAP configuration *IS* superior to CPAP.

Jason, LankyLefty27 has done APAP a real disservice by comparing flawed comparisons.

He compares APAP to his properly configured CPAP configuration. The thing is: his APAP minimum pressure configuration is always 2cm lower than his CPAP configuration. So of course APAP will perform poorly when the minimum configuration is too low. This comparison is disingenuous/dishonest.

Here's a thought exercise.

Let's say your optimum CPAP configuration is straight 10 cm.

So turn on APAP, set your minimum pressure to 10, and then set your maximum to 11.

In this circumstance, how possibly could APAP be performing worse than the CPAP? It can't. It will be delivering at least the pressure that the CPAP was, and should the algorithm determine an increase is necessary, it will increase it.

And no, don't interpret this as a hit piece agsainst Jason. I like Jason, love his videos, enthusiasm, humour and general knowledge. He's just wrong on his condemnation of APAP, which is always a comparison of a misconfigured device.,

Thanks for the explanation. I also think APAP is better than CPAP as it can vary automatically inside the min and max pressure configured. However, my most important question is: Should I let APAP configured from 4cmH2O to 20cmH2O and trust it will only deliver what I really need? Or it is better I set min and max more closely?
As for reference, I'm using APAP for 20 days and I never had a pressure less than 8.9 and more than 10.7cm. So I had set now it as min 9 and max 11. Is it a good approach or you recommend let 4-20cmH2O?

Absolutely not.
4-20 is the very definition of a horribly misconfigured APAP.

I still smile fondly every time this comes up. My former sleep doctor, who was very good otherwise, would get visibly aggravated when I told her I would not use 4 - 20. I had done enough experimenting to know that I needed a minimum of 10.

The only other point we clashed on was foam cervical collars. She did not approve! After she left, my first visit was a year later with her assistant. I mentioned wearing a collar. She said, "Yes, that's good thing to do." :lol:
