I would suggest trying one of the masks with a nasal cradle instead of nasal pillows first. For Example: AirTouch N30i, Airfit N30i, Dreamwear, Wisp, Solo, etc. These masks have a wide, soft, cradle shaped pillow with a single long slot and they rest against the outer edges of your nose instead of directly on the nares. An inner nostril cartilage bump shouldn't interfere with these masks at all.
This type of nasal pillow may work without having to undergo even minor surgery. If not, the surgery is still an option. Worth a try.

Yeah I am using a Dreamwear nasal cushion at the moment (N30i leaks like crazy) while I wait months for the actinic keratosis cut to heal. It seems to be irritating the area just below my nose. Tried DreamWisp and it gave me a pressure sore on the bridge of my nose. I've learned I need to have multiple types of mask around so when one causes problems I can switch to another to allow healing time. Really want the nasal pillows option as that has been my most successful mask (Aloha) in the past 8 years other than the bump pain problem.
Dreamwear gives me great leak numbers but I wake up feeling exhausted. I think it is waking me up with leaks multiple times a night and I fix them and go back to sleep. On OSCAR I see lots of large leak spikes that quickly and suddenly drop to zero leak which look like me fixing leaks multiple times a night and not remembering it. With other masks I often sleep through them and feel better in the morning if the large leak percentage isn't too high. But I think Dreamwear leaks wake me up for some reason. One night I did remember constantly fixing them but usually I don't. I just wake up tired.
