Can I set an AirCurve 10 ASV like my AirSense 10 Autoset

Post by mannme1 » Wed Jul 24, 2024 9:11 pm

I got a used AirCurve 10 ASV from someone who got a new machine from their insurance. It has 2811 hours on it and seems to function fine but I'm having trouble getting it set up like my AirSense 10 Autoset. I have been using the same settings for more than 5 years and I think I'm happy with my results. My doctor certainly is. I tried setting the AirCurve to CPAP mode Set pressure of 16, start pressure of 4.0, ramp time of 5 minutes. For some reason it seemed like the machine was more aggressive than my Airsense and it even seemed louder. Is this normal? If I want to try it in another mode, like ASVAuto, what shoud the Min and Max settings be for EPAP and PS? Start EPAP? Or should I just sell the machine to someone who needs these functions? Since the cost even with insurance paying half is very high, I thought it would be awesome if I had this machine as a backup and maybe even replace my current machine which is almost at the end of it's "life expectancy." I appreciate any insight and advice. Thanks.

You might get close with the settings in terms of setting the ASV machine to function like the AutoSet.
I am not sure that the PS setting in AutoSV mode will do what you want or need though.

Tell us exactly what settings you were using on the AutoSet....mode and all settings.

You will need for PS minimum and PS maximum to equal the same setting and that's where there might be a problem.
I don't know if it will let you set PS min and max to equal the same setting. It may want a range.

I took photos of the AirSense 10 settings from the technician menu and put them in my post. Are there more that I am not aware of? Mode is AutoSet Max Pressure is 16.0 Min Pressure is 4.2 Full Face Mask Ramp Time 5 Min Start Pressure is 4.2 EPR is On, Full Time at level 3 Humidity level is 4 Standard tube no AB filter Essentials is On SmartStart is On

So these are your AutoSet settings?

Pressure 4.2 minimum and max is 16?
EPR at 3 full time

To use AutoSV mode and have it function like the AutoSet

Minimum EPAP 4.2
Maximum IPAP 16
PS minimum 3 and PS maximum 3 (but I don't know if your machine will let you do that or not)

Are you aware that with the minimum starting at 4.2 (and when using ramp) that the EPR does not do much since the machine won't go below 4 cm. So that EPR at 3 doesn't really do a damn thing until your pressure goes from 4.2 to 7?

After 5 minutes of ramp the machine went straight to 16 cm when in fixed cpap mode. No wonder it felt a lot more "aggressive"...it was a LOT more pressure than you were using with the AutoSet.
It went from 4.2 to 16 in one fell swoop....and yes at that pressure any machine will be a bit louder.

From the Aircurve 10 ASV clinical manual:

In ASV or ASVAuto mode, PSmin can be 0–6 cm H2O; PSmax 5–20 cm H2O

So I'd try the settings Pugsy suggested, but you can't have PSmin = PSmax = 0. My feeling is that the PS will stay low unless the machine sees a need to increase it, eg when you have a central apnea event.

Okay, I'm trying ASVAuto but Max EPAP only goes to 15 not 16. I set Min a PS to 3 and Max PS defaulted to 8 and won't go lower so I left it there. If I understand what Pugsy said the settings on my Airsense 10 Autoset of EPR = 3 was not really doing anything? I'll let you know how things go with the Aircurve.

Thanks for your help!

FYI the max pressure can be up to 23 (Max EPAP 15 + Max PS 8 )

There may or may not mean some aerophasia.

You can try to work backward and subtract min or max PS from your Max Pressure.

min EPAP 4.2
max EPAP 13 (16 - 3 min PS ; your max pressure here will be 21) or
or max EPAP 8 (16 - max PS 8 ; max pressure here is 16)
min PS 3
max PS 8

Good luck in your experiments.

That's not what Pugsy said.

"Max PS defaulted to 8" doesn't mean you can't give it a higher number.

My bad, I meant for his specific setting the Max Pressure/IPAP will go up to 23.

The ASV will go up to 30 cm h2o for IPAP.

You can use this machine in straight CPAP mode if you examine your charts and determine what your good pressure is... obviously not 16cm, right? We use Oscar (www.sleepfiles.com/oscar) to determine this... as has been noted, the Median pressure is probably a good place to start.

IMO you should sell this machine and get three or four Airsense 10 Autosets... you don't need ASV therapy and this machine is not going to give you an easy setting that mimics your Autoset. It's worth a lot more than a used Autoset. ASV is for a different kind of OSA, you don't have complex sleep apnea.

Hi again. For two nights I used the machine with the following settings:

Min EPAP 4.2
Max EPAP 8
Min PS 3 Max PS 8

These settings that GrumpyHere had explained worked great. However, I think I’m going to take the advice of vandownbytheriver and sell this AirCurve with less than 2900 hours on it and look for an AirSense 10 Autoset like I already have. Someone who really needs all the settings available on the AirCurve should use this. Thanks for all the great advice and explanations everyone!!
