I think that I'm very very close to achieving optimized therapy, because I can see that the flow rate graph is getting smoother and smoother. I'm going to keep increasing by 1 until I hit the max of 20 or I feel an improvement, subjectively.

I'm using the N20 nasal mask with mouth tape, and I am getting leaks, presumably because I'm not getting enough pressure; my lips squirm around as I'm sleeping bc I'm not getting enough air, so that disrupts the tape, causing leaks.

What do you guys think?

That's not even close to high pressure.

Changing pressures every day will not prove anything. You need to give each change 4-5 days to see if it's working before you change again.

And work at improving your taping: longer strip, firm pressure after you put the tape on, etc.

I've never heard of anybody going about it that way before.

Set your machine to auto with min pressure 10/max pressure 20. Post the chart.

If you really want "high pressure", then get a bilevel, they go to 25, or a iVAPS/COPD version of a bilevel, they go to 30!

Could you explain what you mean by smoothness of the flow rate graph, and why you think a smoother graph indicates optimized therapy?

Also, it'd be good to know why you are using fixed pressures and no EPR.

I notice that two of your charts show a fairly short usage time. Were you sleeping without the machine for part of the night?

You need to have at least 3-5 days of data at a given pressure before you can really conclude whether it's actually doing anything or not.

And you need to realize we all have some really great nights (sleepwise) where we feel good the next day, even if the data's not particularly great. And we all have some really bad nights (sleepwise) where we feel lousy in the morning even if our data looks fantastic.

Your leaks have gotten worse as you increased the pressure.

So in my opinion is more likely that too much pressure is causing air to get into your mouth from the back of your throat and that excess air causes your lips to squirm around and fight the tape and open up just enough to let the excess air out, which causes the leaks.

If you're still not feeling rested, I'd suggest that you look at the usage pattern and your sleep habits as well as pouring over how smooth the flow rate graph is.
