The lip balm creates a barrier between your lips and the adhesive, and folding them in protects them from the adhesive too.
by folding you mean rolling them in?! I do this anyway because otherwise it won't keep tight during the night.

None at all.
I too use the Medvance tape happily. And with this tape, I don't even need lip balm.
Concerning Amazon: since Herbert is in Berlin, his Amazon probably defaults to amazon.de.
But he can change it to amazon.com I don't think that caused the problem.
