Help deciphering my data

I have not been sleeping well lately. My Oscar data looks ok to me but I am no expert. Does anyone see anything that looks like it might need adjustment on my machine (Airsense 10)?

Hi Mr. X:

I am not the best person to offer advice but I think I can tell you some of the questions you will be asked.

What mask are you using?
Please show your "Flow Limit" graph, that is the first thing they look at.
What is causing you not to sleep well? Is it breathing or physical discomfort?
How long have you been using CPAP?

Your leak rate looks very good. Is your mask bothering you or too tight? Your chart looks good to me but I am not a year into the program so I can't really advise. However there are a lot of people in the Zoo that can.

Two breaks in therapy seen on that report...why? Do you know why?
Were there other times during the night where you woke up but didn't turn the machine off?

What exactly does "not sleeping well lately" mean? Lots of wake ups? No energy?

Nothing of great excitement on the report as shown. Might be nice to see the full leak graph and the flow limitation graph but I doubt those will hold any real answers.

So....usual questions.
Medications? Taking any? If so, what?
Detailed description of the unwanted symptoms you are having.
Any other physical or mental health issues?
Pain?
Waking often? Why?

Thanks for the feedback. The breaks in therapy are me getting up to go to the bathroom. Not sleeping well means I am tossing and turning and waking up every 2-3 hours. Only medication I take is Simvastatin. No other physical or mental health issues. 53 year old male. I have been using CPAP since 2010. I use a Fisher Paykel Flexfit full face mask. Flow limit graph enclosed.

You've been on cpap since 2010. Have you always felt like your sleep quality was a problem or is this something new?

Has anything changed in your routine? Changed meds or dosages maybe.

Statins are know to cause sleep problems in some people.

Do you have to go pee fairly often during the day as well?
When you go pee during the night is the bladder painfully full or is it more of an I'm awake and might was well go pee?
Any issues with prostate enlarging.

You know it's normal for people to wake up after the completion of a REM stage sleep cycle. So the waking up part isn't all that unusual but normally we just roll over and go back to sleep and don't remember it.
When you wake up how long does it take you to go back to sleep???

As far as your report...and the FL graph...it's a bit active but not horribly so. I have seen much worse but to give you an idea how FL graphs can look when not much is going on. This one is an old one of mine.

Now I don't know if your FLs are playing any sort of factor in your sleep issues but it's worth trying something different just to see if it helps or not.
Try increasing your EPR to 3.
No other changes at this time.
Sometime flow limitations can cause sleep disruptions...assuming the FLs seen aren't related to nasal congestion.
Are you having any nasal congestion? If you are then we have to address the FLs in a different manner.

There's 2 kinds of insomnia..
1...sleep onset insomnia which is trouble falling asleep
2...sleep maintenance insomnia which is trouble staying asleep. Waking prematurely or often and sometimes experience difficulty in falling back to sleep which ends up being sleep onset insomnia.

Medication side effects are one of the first things we look at when we have trouble falling asleep or staying asleep.
Then we look at other potential culprits and often have to do a lot of detective work.
It's not always easy....not easy to figure out the cause of the problem and sometimes not so easy to fix the problem IF we can figure out the cause.

So about how much time do you think you are awake during the night?

I don't know if adding in more EPR will help or not but it is something that can be done that might help....stress the might part.

Have you talked to your doctor about this problem? Have you considered taking some sort of sleep aid? Either OTC or RX?

I have the sleep maintenance insomnia problem myself and I know the cause and it's still a struggle to get enough sleep even with sleep aids. It's a bitch.

Thanks so much for the detailed reply. Nothing has changed in my routine. I do have nasal congestion/sinus issues but have had those for years. When I go pee at night its because I really have to go. Probably time to get my prostate checked given my age. When I wake up it takes me probably 15-30 min to go back to sleep. I have tried medical marijuana for sleep and it works great but I don't really want to have to take a drug to sleep long term.

Should I still change my EPR to 3 given the nasal congestion issues?

Yes, go ahead and try EPR at 3 and see if it helps or not.
At this point it is the only real thing that I can think of anyway.
