Transitional - Came Back

Hey guys - I was improving a ton this last month (about 45 days) with my AHI around 2 or less and my O2 levels occasionally not dropping below 90 and only rarely below 85.

Last night tho might have been the worst night yet. Out of no where. I have attached Oscar pictures of 10 min spans just as I fall asleep. You can see I literally stop breathing. This happens every night but usually I sleep though it once, goes down to 88% or so and I am okay for the night. Last night I woke up a dozen times so like this.

Does anyone else's Flow Rate look like this?

Happening again tonight. Ugh. So good for over a month. I had my life back. Anyone have any advice for how to keep breathing during these sleep-wake transitions?

This is 15 mins of tonight. I wasn't fully asleep for any of these 15 mins. Every time I woke up 2 mins after "falling asleep" Added pressure did not seem to help at all.

Like SWJ which is sleep/wake/junk and refers to flagged events shown on the software reports that were flagged when we were awake or half awake. Unless we are asleep...the flagged events don't count and the machine doesn't know if we are asleep or not.

Yea its not really Sleep Wake Junk. Its Right at that transition. I lose consciousness but but barely. I can't control my breath at this point. It does it automatically. As soon as I actually get to sleep, all good.

You need to be getting with your doctor about the effect that these sleep onset transitional apnea events are having on both your sleep quality and your O2 levels. It's normal to have some transitional sleep onset central apneas but normally they are brief and they don't impact things all that much and we proceed into sleep and go on about our sleep without a problem or further problems.

There's really nothing you can do about them in terms of tweaking the settings with this machine. Especially when the transitional apneas are central...pressure won't fix them or prevent them.

Yup. Unfortunately Pugsy that's what I figured. My last doctor said "Yea, I don't know why this is happening either", but I am finally getting a CPAP Titration Lab Test in two weeks. Last time I had a No CPAP sleep test this didn't happen. It happened for weeks then stopped for 40 days and now its back. So I am hoping this happens at the Lab Test. If not, I don't know how to fix these.

Anyone who has this transitional stuff have any suggestions on dealing with it?

Unfortunately it's really hard to fix a problem when we don't know what is causing the problem.
Even more difficult when the problem is transient and doesn't happen all the time.
I am sorry but I don't have any answers. The human body doesn't always adhere to the script that makes it easy to rule in or out potential culprits.

That’s okay, thanks Pugsy. You help a lot of people on this board, including myself. That’s the frustrating / scary part about this. No one on any board or any doctor I’ve met can make sense of it. I’ve working out hard to get in better shape (wasn’t that bad before) so hopefully as I keep losing weight it will improve. If anyone has any suggestions please don’t hold back.
