Help needed with understanding my results

Post by Vilpi » Fri Jun 26, 2020 3:48 pm

Hi Everyone.
I am quite new here. Last year I found I have apnea based on this test result (this has been translated to English sorry if it is not well done)
Description: Technically satisfactory registration. Analyzed time from 01:00 to 07:50. Total analysis time 6 h 34 min (respiratory airflow), of which the patient slept 24%. Respiratory index (AHI) overnight 32.3 / h, supine 26.5 / h, in other positions 34.4 / h. 48 apneas. The initial oxygen saturation is 98%, the average oxygen saturation during registration is 95%. Lowest oxygen saturation 88%. Less than 88% values 0 min. More than 3% decreases in oxygen saturation (ODI3) 35.8 / h. Quite a lot of snoring.
Now I have been using the CPAP machine regularly most of the time during past 6 months. This is the report I produced today.
Can anyone help me to understand how I am doing and what is the situation with me? Or if I need to change something?

Thanks in advance for helping me out.

Your leak management needs some serious improvements. It's bad enough that the machine could be missing apnea events so that nice low AHI might be inaccurate.

What mask are you using?

I am using AirFit™ F30 Small size. I tend not to wear it very tightened as I cannot sleep when it pressures my face too much. The thing is I feel I need the medium size to feel more comfortable but I need to wait until November this year to get a free one from our NHS.
Can you elaborate more on the leakage? How can I know it is leaking too much or if it is normal? I am asking from the report point of view.

See that red line at 24 L/min on the leak graph? That's the line in the sand where large leak territory begins. While it is a very conservative line in the sand....the further you go above it the increased potential for the machine to run into trouble sensing, recording and responding to apnea events. You have more nights where you are way above it than below it.
Without seeing the actual nightly detailed leak line it's hard to know for sure just how bad your leaks are but based on these numbers...it's fairly significant. From my own personal experience with a ResMed machine....up to around 35 L/min the machine can do a fairly decent job but after that things get really iffy and the further you go above it the iffier it gets.

Now that overview graph is based on 95% numbers which don't always tell the whole story and are easily skewed to the high side but without evaluating each night in more detail we have to assume that leaks are bad until proven otherwise.
95% numbers mean you were at OR BELOW that number for 95% of the night so maybe it isn't really as bad as it looks but based on what is shown above.....need to at least look more closely and evaluate more closely.

Thank you. I understood. Last night I tightened the mask so I could see how the result will differ. Here I have attached the detailed comparison between last night 27th, and the night before 26th. Can you now tell more about how I am doing if the mask is tightened well enough?

The very short segment is fine.
The other longer segment where the median leak number is 20 something...not so fine.

If you would use OSCAR seeing these results would be much easier on everyone.
