More tired after CPAP with almost perfect fit\AHI and duration

Ok been on CPAP for 3 months now, have a DreamStation , set to 12-14lbs BiPaP, been getting 100% mask fit and a almost perfect .1 AHI..plus getting 6.5 tp 7.5 hours per night. All that and I feel WORSE than before, horrendous tinnitus and feel exhausted at 4pm in the evening. I was 21 AHI before got my CPAP and I was much more functional. Been to doc and no sinus infections or anything else going on health wise. Primary care doctor at a loss. The other day I slept 7.5 hours and then took a 2.5 hour nap during the day with mask on and still by 9pm was so sleepy I conked out reading. I was a 4-6 hour a night sleep and had much more energy. Anybody else have this experience?

It cm to water or shortened to just cm and not lbs....12 to 14 cm.

Yes, unfortunately some people do end up feeling worse on cpap than before for any number of reasons.

We need more information if we are to come up with any ideas as to why.

Also....couple of questions for you
1...do you take any medications of any kind? If so, what?
2...is your sleep fragmented with multiple wake ups during the night? If so, any idea why?

Probably more questions later but that's enough for now.

Two blood-pressure meds, a diuretic and an ACE inhibitor. My GF says I sleep like a rock, much less tossing and turning. I literally close my eyes and it's 7 hours later. Before I was up all the time. My doc suspected some kind of bug\virus, but been weeks and that is not the case. I never really did have that "Wow I feel so much better feeling" when I started CPAP. I did it because my GF said I snored like a lumberjack and I was gasping in my sleep. One benefit is I lost 20lbs and kept it off. I just want to feel more rested.

So you are more tired during the day now or more sleepy during the day now or both?

They aren't the same thing.

I would say more sleepy. Meaning when I am sitting in a relaxed environment\position I could sleep.

Maybe there is something on the report that stands out screaming "fix me and you won't be so sleepy during the day".

How come the break in therapy at 23:00?

How come the max of 14? You are running a really tight auto adjusting range....any special reason for doing that and not letting the machine go higher? It wants to at some times during the night.
Your leak line (that top total leak line) is sort of all over the place but your pressures aren't all over the place.
Are you having any leak issues? Dry mouth which might mean mouth breathing? It's not horrible and you aren't hitting large leak territory but it is enough that it could be impacting sleep quality and anything that impacts sleep quality in turn messes with how we feel the next day.

I don't see anything standing out screaming "fix me" though. I just see some subtle stuff that might be a factor...and might also not be a factor.

The 23:00, could not tell you, maybe GF needed to talk to me. The max 14 was set by the sleep clinic.. they recommended. Leak issues, hmm could be. I tried a nasal pillow mask and it just was not for me, I got tangled in the hose, I thought maybe that would handle it. I spoke to the respiratory therapist and she said the leak issues her words.."The machine adjusts for that, important thing is to get mostly good mask fit and to have compliance"

I was able to get into therapy settings and adjust the auto upper range to 15cm(from 14cm)...I'll post my results here in a few days

I doubt if changing the max is going to do much but it is worth trying. You spent very little time at 14 and there wasn't this big cluster of events happen when it couldn't go higher.

I am wondering if something else is going on to disturb your sleep quality or affect how you feel during the day.
While we don't expect overnight miracles with cpap we certainly don't expect to actually feel worse when there are no outwards signs of why we feel worse.

Medications can impact things but I assume you were on those same meds prior to starting cpap.

Do you drink alcohol in the evening? That can mess with sleep quality.

Nope don't drink and my sleep hygiene has been the same. Meaning same mattress, same temperature in room, same meds, other variables the same. I wonder if after 3 months I still having a lingering sleep deficit? Like I may need to bump up my sleep time to 8-9 hours and not 6-7.5. Also try a sleep supplement(s) like Valerian Root, Melatonin, extra magnesium.

You know maybe you do need some extra sleep above what you are getting.
I know if I get less than 7 hours of sleep I can pretty much see a nap or the strong desire for one in my future. I simply need more sleep than I get.
Easier said than done though...believe me I know that very well myself.
I don't know that I hold much into the sleep deficit thing long term but short term I do.
