new ASV not feeling the same as old ASV
I recently aquired the Respironics DreamStation BiPAP AutoSV after about 8 years on the old Respironics AutoSV Advanced. My old settings were 15 IPAP max, 10 IPAP min and 5 EPAP. i told my therapist this and for whatever reason they couldn't replicate it. it would go from 5 EPAP all the way to 15 IPAP, which is pretty aggressive for me to handle. Now i'm on 5 EPAP and 10 IPAP with a max IPAP of 20, and my AHI has been between 4 -6 the past week instead of the consistent 1.3 i would get on my old machine. I'm kind of skeptical of my respiratory therapists since they took a few days and adjustments to get to my "old" settings, but they are telling me i should do a variable EPAP and change from a standard ASV setting to an Auto ASV. I'm hesitant about that since the thing that got me on BiPAP ASV years ago was my severe complex apnea (33 AHI at my first studies) and my intolerance of higher EPAP. also, it's just stressful going through adjustments and tiredness again after 8 years of consistency. I just wanted to update my old machine that had 20k hours on it. i was just looking for some thoughts or input from the ASV crowd around here and if there's something i'm missing.

Congrats, You have found out it's not the same Welcome to the 21 century, New things are different from old things, also it probably is set different, but may not ever be made to feel exactly like the old one. Even two bought from the same build run may feel different, but still they can treat you. What I ran into when I went from my Beastie -- the old Respironics AutoSV, not even the advanced, to the System One 960 is that they changed the way they describe EPAP and IPAP.agentandya wrote: ↑Mon Aug 19, 2019 11:41 amI recently aquired the Respironics DreamStation BiPAP AutoSV after about 8 years on the old Respironics AutoSV Advanced. My settings were EPAP = 6, MinIPAP = 10, Max IPAP = 14.
I ended up with the EPAP = 6, and a MinIpap of 16.
Because now it is described as "pressure support" and not as MinIpap and MaxIpap. And it's added on top of your EPAP. So they had taken my MinIpap and set that as the MinPS and the MaxIpap and set it as the MaxPS, which just about blasted my face out of the mask!
I figured it out, and looked up how to change the pressures in my manual, corrected the error, and crawled back into bed.
So for me, I now have an EPAP range of 6-7.5, and a MinPS of 4 and a MaxPS of 8. (RT and doctor suggested a range of EPAP for me since my new machine had that capability, but mine almost never budges off 6.) So when I add the MinPS to the EPAP, I get a MinIpap of 10, and when I add the MaxPS to the EPAP, I get a MaxIPAP of 14, just like I had before.

thank you - this is helpful and makes sense - i don't even think my therapist understood this much.Kiralynx wrote: ↑Mon Aug 19, 2019 1:58 pmWhat I ran into when I went from my Beastie -- the old Respironics AutoSV, not even the advanced, to the System One 960 is that they changed the way they describe EPAP and IPAP.agentandya wrote: ↑Mon Aug 19, 2019 11:41 amI recently aquired the Respironics DreamStation BiPAP AutoSV after about 8 years on the old Respironics AutoSV Advanced. My settings were EPAP = 6, MinIPAP = 10, Max IPAP = 14.

Because now it is described as "pressure support" and not as MinIpap and MaxIpap. And it's added on top of your EPAP. So they had taken my MinIpap and set that as the MinPP and the MaxIpap and set it as the MaxIpap, which just about blasted my face out of the mask!

Re: new ASV not feeling the same as old ASV
I had the same exact situation. I went from the 960p to the dreamstation and it felt a little different. I felt like i wasn't quite getting the same pressure on inhalation and it took my mind/body a few days to adjust. Everything is great now. Good luck!
Re: new ASV not feeling the same as old ASV
Yes, except I had a couple typos in my original post. (I think I will go back and see if I can edit it.)agentandya wrote: ↑Tue Aug 20, 2019 5:31 amthank you - this is helpful and makes sense - i don't even think my therapist understood this much.
PP should be PS -- Pressure Support.
So my last statement should read:
"So for me, I now have an EPAP range of 6-7.5, and a MinPS of 4 and a MaxPS of 8. (RT and doctor suggested a range of EPAP for me since my new machine had that capability, but mine almost never budges off 6.) So when I add the MinPS to the EPAP, I get a MinIpap of 10, and when I add the MaxPS to the EPAP, I get a MaxIPAP of 14, just like I had before."
I'm glad the explanation helped. It was a real shock when I got blasted with 16 to 20 when I was used to 10 to 14, but didn't take much to figure out. The fact that I had been reading here for almost five years at that point gave me the knowledge and the tools to solve the problem.
Good luck with your therapy!
