Need some help to find right pressure

1) Sleep Study Results
Sleep Disordered Breathing: Severe SDB, with a respiratory event index (REI/AHI) of 31/hr.
The majority of events were obstructive, consisting mainly of hypopneas.
Events occurred more frequently in the supine position (REISUPINE= 35 vs.REINONSUPINE= 20). The patient spent 69% of the MT in the supine position.
Oxyhemoglobin Saturation: : Severely disturbed. Baseline O2 saturation was 93%. The oxygen desaturation index (ODI) ≥ 4%
was 28.3. Mean O2 saturation was 90% and the patient spent 49.6% of study time with an O2 saturation < 90%. Minimum O2
saturation was 82%.
Cardiac: The average heart rate was 58 bpm and the highest recorded heart rate was 94 bpm.
Miscellaneous: Moderate snoring was detected.
Subjective: The patient rated the quality of sleep as worse than usual.
IMPRESSION:
Severe OSA, not only supine-sleep related, with severely disturbed nocturnal oxygenation. (G47.33, G47.10)

2) Doctor ordered AUTO CPAP set at 5-20
3) AHI above 20 for first 2 weeks -sleepy all day still feeling crappy
4) Pressure adjusted to 8-20 for 1 night AHI 11.29 -felt better not sleepy as much
5) Pressure adjusted to 9-20 last night AHI 14.49 -same as above

I was on CPAP 10 years ago for 2 years and taken off by doctor because sleep study showed no apneas.
I want to know which way to go now should I go off auto to where I was 6 years ago to CPAP set to 10 or continue to adjust the auto?
Please help me get the AHI down.
What would be a common sense way to adjust the auto?

Welcome to the forum.

The bulk of your AHI is clear airway/central apneas and more pressure is unlikely to help with those.
More pressure might help with the hyponeas...maybe just a little more minimum pressure.
For the CAs....try turning Flex exhale relief off and see if the CAs reduce any. If you do that...don't increase the minimum just yet because turning off Flex effectively increases the minimum a bit.

Questions for you..
1....Do you take any medications of any kind? If so, what?
2...How is your sleep quality....wake often or sleep soundly for the most part?

Please review this thread for graph formatting....you have included some graphs that aren't really needed and in doing so you have made the needed graphs a bit small. No need to redo these images but I mention it for future reference.

Thank you for taking the time to help me out.
for Medications I take:
Celebrex 100mg
Prilosec 40mg
prevastatin 10mg
Amlodipine 5mg

As far as sleep goes it is mostly straight through. sometimes I am awakened during the night and fall back to sleep within 20 minutes. I get an average of 7.5 hours

How long did it take you to fall asleep last night?
Did you wake up at 5:20 and sort of lay there for a little while unable to go back to sleep?

Took about 30 minutes to fall asleep.
I did wake up at 5:20 and thought I went back to sleep a couple of minutes later until 6:00

The times where the AHI was higher at the beginning and end of the night were when you weren't sound asleep.
So those flagged events have to be mentally removed from the evaluation. They are what we call SWJ or sleep/wake/junk...false positive flags. If you aren't asleep they simply don't matter.
After 5:20....you were most likely in and out of sleep or just half asleep. SWJ happens often in this situation. We are awake more than we realize.

In the middle is when you were asleep and not a lot was happening at that time but there is evidence of a little bit of stuff happening...so I would suggest an increase in the minimum pressure just a little and see if the times when you are asleep that the little bit of stuff will reduce. We ignore the SWJ.
So maybe 0.5 to 1.0 cm more minimum.
