Surviving at 5000 feet

I am a newby , diagnosed with severe sleep apnea in September with an Ahi of 64 at 500 feet altitude. For th last 14 years I have spent 50% of my time at 5000 feet in Mexico and 50% at 500 feet in Canada. My AhI is around 5 at 500 feet and around 16-
22 at 5000. My problem is 98% centrals and brain fog. Need to snooze everyday but generally lead a normal life.
My sleep doc does not have the time to fine tune my settings for 5000 feet so I am looking for help. I am on an aircurve 10Vauto and my current prescription is autobipap max ipap 18cm, min epap=5 cm; delta=4. I am focused in getting my ahi below 5 at 5000 feet.
Recently I have changed ps=4 to ps=0 which has decreased my ahi a little and has reduced centrals from 98% to 70 % with hyps 30% with the odd obstructive. My understanding is that centrals can be reduced by reducing pressure but I am way out of my depth. I need help to achieve my objective. My compliance is 100%, my determination is 100% but my knowledge is 10% but growing fast. HELP!
