Best approach to central.

Back again after another lapse of intelligence. Been using the machine faithfully for 2 months and will continue. Lack of discipline has caught up with me. So that said.......... this time around I am having a lot of centrals. I just had surgery (broke my wrist tripping over the cat) They found my lung was partially collapsed. They also had to keep telling me to breathe. It appears I am a very shallow breather. I think because of my deviated septum, I made adjustments to accommodate it. So I have to start reminding myself to breathe even when I am awake.

So I looked at other topics about central and I just dont always understand the language. But I want to educate myself. I will request another sleep study to see if they will confirm that many of my apneas may be centrals. I wonder if I have the Cheyenne (stoke?).. thing. Need to find out.

Meanwhile, suggestions on the best cpap to get ?(both home and travel ) I have a s9 vpap. At least 6 years old. I am willing to buy outright to get all the features I need. I use the N10 nasal mask with a chin strap or a cervical collar. It is the most comfortable way for me and leaks are rarely the issue. Pugsy once convinced me anything under 5 is still therapy so I am not chasing perfect scores.

So any advice would be appreciated.

Just how many centrals are you seeing? Hourly average?
How about posting a detailed report from a typical night so we can see what's going on?
Are you still taking pain meds for anything?

Even though the S9 doesn't flag Cheyne Stokes Respiration there is a way to look at the flow rate to see if it looks like CSR.

apparently I am just a little slower and dumber with the splint on my hand. I did the screenshot ....and see something has changed in how we share them? No more imgur? So the screenshot is in my documents. How do I upload it to here.

Also, I just checked the last 2 nights. They were quite good. one was 2.46 and one was 1.85 with a pretty even split between central and obstructive. So maybe my issue is improving. Or maybe the cervical collar is a winner since thats what I used last 2 nights. My bone was broken for 10 days before they could read an X-ray and I had surgery on day 11. So yes, I have been on various meds altho I try to keep them to a minimum. I am only on ibuprofen right now. (and a little children benadryl due to allergies going nuts)

no idea how I just did that> I had some nights that was 20 ahi with most centrals. Maybe pain or meds were responsible?

Most definitely either of those could be responsible for the centrals. If they don't come back...don't worry about them.

Often, "a lot" of centrals isn't a significant amount.

And if that screenshot is a typical night, you're are insignificant.

You seem to need more pressure than you've got (based on the fact that your pressure keeps jumping up when you get down too low... and you could reduce your AHi further by raising your minepap. You'd likely need to raise the maxipap.

I dont know all the common lingo yet. My settings are 16/11-3 Do you mean raise both the 16 and 11? Incrementally or a whole point?

Your settings are 14/11 to 16/13 I'm suggesting that you set MinEPAP to 12 and MaxIPAP to 18, leave PS at 3 for now, but you may need to lower PS, and may need to further increase MinEPAP, but the above is a start.

Those are what the settings in your machine are called.
