Advice needed

Post by banjosnore » Sat Jun 16, 2018 9:36 pm

I've finally got the P10 masks and heated tube which yielded 5-6 hours of sleep when I first used it during my CPAP trial. However, I've been diagnosed (directly quoted from my ENT) with:
"nasal septum is somewhat irregular and the inferior turbinates are enlarged and quite vascular."
Also during my sleep study, I have a high 41% flow rate limitation without snoring which causes my fragmented sleep. Directly quoted from my sleep study:

"There is snoring noted, significant airflow limitation with frequent cardiac variations suggesting sleep fragmentation due to
upper airways resistance.
%Flow limitation without snoring: 41% "

I've been prescribed with Butacort to reduce the swelling. However, since the diagnosis, I've been noticing that my right nasal is becoming frequently congested regardless which position I sleep to.
Now that I've got the correct gears that helped me last time from my cpap trial, want to determine the efficacy of my Resmed airsense 10 with P10 and Heated Tube by itself so that I can report with my ENT that we've exhausted all option.
I am highly likely need to get my turbinates reduced if all else fails which I prefer not to do until I I've tried all non-invasive options but I'm hoping that if I approached this scientifically. I'd get the correct settings without a Butacort.

In your current/previous newbie experience, Do you think that the following testing is sufficient?
* 2 weeks on with Butacort using a predefined settings. Keep an eye of the flow limitation and awakening arousals.
* 2 weeks off without Butacort using the identical settings with tuning (likely humidity settings and pressure settings which I think I've already optimized).
* 2 weeks off without Butacort with the tuned settings.

IIRC Butacort starts working after 5 days of consistent use.

Butacort is a cortisone spray. It's not available in U.S., but we have equivalents.

My doctor wanted me to use a cortisone nasal spray for six weeks to see if my turbinates would shrink. This is done to prevent unnecessary surgery on turbinates that are swollen due to inflammation.

The doctor and I both thought they would not shrink, and they didn't. The surgery was one of the best things I ever had done for myself.

I would not worry about the CPAP therapy. If your turbinates need reduction, you will benefit greatly from it whether or not you have sleep-disordered breathing.

Thanks for the response! and thanks for that info that corticosone can shrink turbinates. My ENT didn't mention that. Yesterday, we both agree that its likely that I'm going to get a surgery and I have done the right thing to attempt the CPAP tasks and especially with Butacort this time.
Then I suggested that I will still try to use the butacort + cpap combination over the next 4 weeks and if this doesn't work then I should have a surgery.

Last night, I had quite a bit of arousals,(more than twice) that I turned off the CPAP cause I had a bad sleep the other night. However, I'm yet to check previous sleep theraphy if this is due again to the flow limitation but it is likely the culprit which pugsy has been pointing out.

It's been a hit and miss lately and it mostly points to the flow limitation but I cannot add another assertion this unless I have a "oximeter?". Maybe that may yield the same "spikes" of heart rate based on my previous study?

So far, I've
1. I've cranked up the humidity to the max.
2. Have a room temperature at 20C so I don't experience a rain out.
3. Been consistently using Butacort consistently this time. Its my 3rd consistent day.
3. Min. Max pressure remained the same cause my previous graphs indicates it looks like the optimal number.
4. Been using a pillow masks since my last posts which I have lower leak rate than the nasal masks.

I hope it works. If not. Surgery :(

The purpose of the humidifier is to prevent drying of the nasal passages. In some people, high humidity levels cause nasal congestion. A humidifier setting should be chosen high enough to prevent drying, but no higher.
