CENTRAL APNEAS QUESTION

Post by Ms Piggy » Tue Oct 17, 2006 1:37 pm

Does anyone know if the setting on your machine can cause/encourage central apneas. I switched to apap on 13th and have had 2 apneas and 5 centrals since. First night ok but It was a bit wild the 2nd night, pressure went way up, nearly blew my breeze off! so leaks were a problem. I had IFL1 on and 2 off. This was reversed after the 2nd night.
Centrals were not mentioned on my tritration june 2005. Maybe this amount of centrals is an acceptable level, I wouldn't know.

I have read in the following article http://en.wikipedia.org/wiki/Sleep_apne ... leep_Apnea
" some people with sleep apnea have a combination of both tpes, when obstructive sleep apnea syndrome is severe and longstanding episodes of central apnea commonly develop." and that the use of a cpap machine is not effective treatment for them.
Although I was only diagnosed last year I have had the symtoms for some time but they went unrecognized until I saw an article in a magazine. My Dr. didn't clue in at all. The Sleep Dr. regards mine as severe. I don't want something else to go on untreated but of course i dont want to panic every time I read something that could apply to me but doesn't necessarily apply.
The Dr is at the clinic tomorrow, once a week, so I may do the 5+ hrs return drive, they seem to want me to do but then they are always covering themselves - just in case.
If you know something about centrals I would be interested to hear.
thank you.

info from Ms Piggy MISTAKE - I HAD one central of 0.3 on my tritration, whi ch lasted barely 3 hrs due to a badly fitting mask full face mask.
pressure tritrated 7 machine set 6 - 10 & 7
switched from basic remstar to PB420E apap
2ND NIGHT, the leaky one
Apneas 1 central o.3; 2 Hypos 15 hypops FL 5 acoustical vib.5
the total of the other 3 nights is
apneas 1 central 3* hypops 5 hypopFl 0 " 0
* one at 0.3, two at 0.1

apart from the centrals it's ok,
I just don't know if I should be concerned about the centrals.

I wouldn't be concerned about a few scattered through the night, either at the study or at home using cpap. I'm not a doctor, though.

Thanks for the comments, one other question though, do the 4 central apneas have any significance?

None whatsoever.

the central may be a fluke, but it could also be in response to a pressure increase, I would reset your pressure to:

Min=6.0cm
Max=8.0cm

(your last titration of 7.0cm is right in the middle) and see what it does the next few nights. You should study that report carefully and see if the central is associated with a specific pressure (i.e. like it appeared after machine ran to 9.0cm or 10cm for a few minutes and the central was seen corresponding to it). If it appeared at the 6.0cm pressure setting then there is no need to make any change at all.
