Newbie to CPAP

Post by Danlo67 » Tue Dec 19, 2017 11:52 pm

Hi all. As the subject suggests, I'm a newbie to CPAP, a one night user in fact.

I have a Phillips Dreamstation with the new gel nasal mask without the pillows. It's set to 9 cm H20 on a ramp of 4.0.

My sleep study report showed my AHI at 46.3 without CPAP. On my first night last night I got a a reading of 25.5 AHI after 5.6 hours.

Just wondering if that's a good result for first night or should I expect a little less than that?

Also wondering how long I should be on it before I start seeing some significant results?

Love to hear from all you oldies to CPAP what you think?

Thanks for reading

A treated AHI of 25 is not great. It maybe you need more pressure or something. Download sleepyhead and post up some graphs and we can see what’s going on.

Sorry but no can do at the moment because I'm still on the 6 day trial thing and dreammapper won't let me Bluetooth it for some reason. Bit of a bummer really

Were you awake a lot during the night? That wouldn't be unusual in the beginning. Having lots of wake time can make the data not reliable as the machine is including wake time breathing irregularities in that AHI. If as your sleep gets better your AHI goes down, you can figure wakefulness was skewing your data. One night recently I masked up then someone started a conversation and I joined in. In an hour of wakefulness my machine showed an AHI of 17. Had I not remembered the talk time, I would have been concerned. If you felt you slept well during the night, that would be more of a red flag for me. If you see these numbers a 2nd night I think I'd want the provider to look at things now rather than wait for 4 more nights which will put you in the holidays and provider availability may be limited for assessing your data and making changes. Is your provider monitoring your data during this trial period?

Yes. I got a call from him half an hour ago. He got the data so he knows the reading. He didn't say anything about it. Just wanted to know if I was comfortable with it. I told him yes but I found it difficult to move. He just said you get used to that. I asked if it was OK to try the pillows. He said he'll get my person taking my case. (Whatever you call them?) to organise them for me.

Sorry. I didn't answer. I took about to actually fall asleep. So I think. For my sleep study I had delayed rem. 124 minutes. I don't know what is normal?

REM is normally around 90 minutes after sleep onset. So 124 minutes after sleep onset isn't horribly delayed. Mine was 235 minutes.

Your AHI from last night is way too high for sure but it's the first night and often there is a lot of awake breathing (with mask on) that gets flagged by mistake because your machine doesn't have a clue if you are asleep or not. It just measures air flow and our air flow/breathing while awake is very irregular when compared to asleep breathing and it's quite common for awake breathing to cause the machine to flag events that aren't necessarily real.

So I wouldn't panic over that AHI of 25 immediately. We like to see it less than 5.0 though. So give it a bit of time and if the AHI is still high once you go to sleep quickly and stay asleep for the most part then we wonder about the settings being optimal.

What is the model number of your machine? Model number is on the bottom or side and starts with DSX and then 3 digits and then some letters.
Need to know those 3 digits.

Wish you could use the available software so we can see when those events got flagged and have a better idea if you were asleep or awake when they got flagged. Can't see it using DreamMapper.
