Need advice Pugsy
For a week been using Dreamwear size small cushion/pillow nose gear. Finding now its slipping out of nose right away. No smaller size. What would you suggest as an alternative. Must fit Dreamwear mask. Appreciate your help and TY

I assume you are using the medium frame that comes standard with the DreamWear cushion/pillow packaging.
You might try a small frame. I found it made a substantial difference in mask stability.
A small frame is available separately.
I do think that for some people an extra small in the nasal gel pillows would have been a nice option. I think I would have a least like to have tried one. I think I would like an extra small better...but we don't have that option.
So you might try the DreamWear with the Small headgear/frame. It might just be all you need.
If you think you are real close with the DreamWear and still using the medium frame and didn't know about the small frame...by all means try the small frame. I didn't think it would make THAT big of a difference but to my amazement it did.
I don't know what to offer as a substitute because I don't know what masks you have already tried and I know you have tried several and I don't know why the ones you tried failed so it's hard to know what to offer as a fix for a problem if I don't know the problem.
I really only have experience with nasal pillow masks.
The only nasal cushion (over the end of the nose) mask that I ever liked was the ResMed Swift FX Nano. The plus with the Nano is that you can also use the FX nasal pillows with the headgear.
In terms of nasal pillows these are the ones I usually recommend people try if nasal pillows is what they want. In no particular order. Obviously for me the P10 is my preferred mask.

Swift FX Bella ...no headgear just straps over the ears
F & P Brevida
F & P Aloha
F & P Pilairo...very similar to the Brevida...I think the Brevida is better because of size of the pillow.

Tap Pap nasal pillow ...no headgear at all..uses a mouth guard to stabilize the pillows...and I used this mask for a year
all masks have pros and cons...all of them...nothing is ever totally perfect for everyone.
We are all individuals and have individual personal preferences and needs and my preferences might be totally different from someone else's preferences.
If you think you like the DreamWear for the most part ...try the smaller frame.

Obviously I'm not Pugsy. I hope that is okay. (oops! I see she has posted while I was writing this, but maybe this will still help. Good luck and best wishes!)
What size headgear are you using, and have you looked at their fitting guide to know it is sitting correctly? The side strap should lie about mid-way between your eyes and ears. Too close to either and it is the wrong size (I think too close to eyes means it is too small and too close to ears means it is too large - but do find the online fitting guide and double check; my memory sucks).
I saw someone mention (using different gear) that they used a stretch woman's headband, folded in half, and having it lie across the 'stems' thereby keeping the nose-thingies in place. I don't know if that would work in your situation - I've only used the Dreamwear cushion without the gel things.

Heres the thing. I have a med. size (says med. on the top) Dreamwear mask,but the theapist told me its really a small. HELLO--is she just blowing smoke or was it marked wrong. If she tells me again the medium is really a small, is she just bS ing me? Anyway have to go there today. Nothing else fits with DreamWear but DreamWear so I will bring this mask size thing up again. Right now the straps are adjusted almost as tight as they can go.

She's blowing smoke...if the frame says medium on it...it's a medium and not a small.
All the DreamWear packages come with the Medium frame. Smalls are separate purchase.

I noticed in one of your other threads that you slather on k-y jelly, perhaps that is making the pillows slip and slide. Most of us recommend a small bit of lubricant, not lots. We tell folks to just rub your finger "'lightly" over the tube end, then gently rub this tiny amount around the nares. If your pillows are kinda slippery to the touch when you take them off in the morning, you have too much on there. Also. if you think that the regular size head gear is too big as Pugsy suggested.. try this before you buy the smaller size.... take a tuck of fabric of the back of the strap and gently hand sew it together, then you will know if the smaller size does the trick. This therapy is perfected (if it can be perfected ) with a hint here and a hint there by others who have already "been there done that" ideas. We can only get it dialed in the best way possible by trying everything until we find what works for us, no mask is perfect but to get a good nights sleep with the least amount of apneas it takes work.

I use the Dreamwear and I don't understand the need for lubrication on the mask or nose.
The pillows actually have a very small contact area and what I think is a light touch. Even though I am primarily a stomach sleeper and tend to change head position frequently, I have not had a leak problem. Yes, I get some small leaks for short periods of time, but it's nothing that would affect therapy.

Altering the back strap isn't going to give much help in figuring out if a smaller frame will help with stability issues.
The issue with the frame is the circumference of the silicone itself.
Been there done that with the medium frame and the back strap.
It was only after I got the small frame that I could really notice a difference in stability.

I wonder if the therapist is trying to say small cushion = small mask (ignoring the size of the headgear). This could just be a miscommunication. I sometimes find it hard, Amess, to understand exactly what you are asking. No offense intended. Maybe it is a misunderstanding rather than trickiness on the part of the therapist.

Max--gel in the nose to prevent drying. My score on my air res med as for leaks has been higher than ever. Leaks 49 on one day--29 on the other Limit on res-med 24. If over that adjust mask. I think the whole set up (mask and nose) is too big.
