Clustering of Obstructive Apneas

Post by Kismet » Thu Sep 21, 2017 1:15 pm

After reading some of the comments and posts regarding the Resmed AirSense 10 for Her I turned the "for Her" off and went with just the APAP. It had looked like the "for Her" feature was not allowing the pressure to go over 12. Since changing the machine to APAP the pressure is going over 12 but I seem to be getting more events. The attached chart shows a night where I got a number of events grouped close together with obstuctives lasting more than 30 seconds for a number of them. Any suggestions? Thanks!

Possibly "positional" sleeping.
Do you happen to sleep on your back at any time? That's sort of what it looks like.

Clusters of obstructive events are usually caused by your chin tucking down towards your chest. Use a thinner, firmer pillow and it is often recommended to get a soft, neck brace in a loose fitting around your neck which usually costs around $10 at Walmart or a drugstore. Not the stiff kind used for neck injuries, this is the softer kind and make sure it is not too tight. That would help to keep your neck aligned and they are often useful to prevent the mouth dropping open causing mouth leaks.

My bet would be REM stage sleep ...maybe with a bit of sleeping on your back thrown in.

I see this kind of stuff often since my OSA is 5 times worse in REM sleep. It happens if the minimum isn't quite where it can get up to where it needs to be to better prevent the airway collapsing.
If this is something you see every night then maybe just a little more minimum pressure to give the machine a little better head start in getting to where it needs to be to better hold the airway open.
If this is something you see maybe once every 2 weeks then I don't know that I would do anything about it.
Up to you...how you would want to tackle it. I would just use more minimum if it were me. I don't know that I would want to wear a cervical collar all night to maybe deal with 15 minutes of ugly once a night. I suspect 1 cm more minimum would clean things up quite nicely or at least break up the cluster so it wasn't so ugly.

This is probably what you were seeing in the for Her mode and the machine wouldn't go high enough.
