Letter to my sleep doctor:

Hi Dr. Xxxxxxxxx:

I have been faithfully using the new machine (S9 VPAP ASV) for more than a month (7 - 8 hours every night) and am consistantly getting an AHI of 0.0. But I don't feel any different - tired, groggy, unrefreshed, grumpy, cognitive difficulties.
I would like to get an overnight oximetry study to check oxygen levels at night.
I would also like to get a complete copy of the last PSG, not just the summary report, so that I can have it reviewed for clues as to why I continue to feel this way.

Hi Mairtin,
Good job with excellent compliance with using your device. I have put the order in for overnight oximetry for you. Do it with use of the device for the whole night. You will be called by staff here to schedule it, or you can call us and get it scheduled. About getting the raw data of your sleep study, unfortunately I have been told by the director of the sleep lab that we do not have the capability to transfer these very large files onto other media. The report is what you already have. I can get you "tables and graphs" of the study, which is basically same numbers and data in a tabular and graphical form.

If you are successful in getting an oximetry study pay particular attention to your pulse rate.

It might also be a good thing to have a cardiologist take a look at the pulse rate data.

Since you are using xPAP therapy, I would expect your O2 levels to be OK, but your pulse rate changes may reveal arousals caused by something other than O2 desaturation.

How does that work? What does the pulse rate have to do with sleep apnea? Can you point me to information or studies about this?
I have PVC with bigeminy but my cardiologist doesn't think there is any connection between that and how I feel.

Arousals are frequently accompanied by an increase and/or a change in pulse rate. Unfortunately, normal pulse rate fluctuates during the night, so an unusual pattern needs to be recognized by someone that has experience in reviewing pulse rates.
