I've been asking a few RRTs and RPSGTs about this. Of course, none of them feel comfortable advising me about my specifics but - I did find out that an RRT's fine tuning a machine's "comfort options" is not changing the prescribed pressure therefore it is NOT against the "rules". I also found out that a lot of RRTs are not comfortable doing so BECAUSE they don't understand (altho they SHOULD) the differences between the major brands AND/OR just what each option does. Its not enough to be "just" an RRT when it comes to sleep and bi-level therapy comfort levels. SO MUCH FOR THE RRT ASSOCIATION'S WRINGING THEIR HANDS OVER POSSIBLE LEGISLATION THAT WOULD REQUIRE EXTRA TRAINING FOR RRTs FOR SLEEP and having to pay for that training!!!!! A sleep tech provides the data and starting point, the sleep doctor interprets and dictates the results and writes the equipment order (script) and pressure settings and a respiratory tech is supposed to fine tune the therapy as needed. WHO is NOT doing their job here??? The RRT!!!

Inspiratory Time (known as TI) is the total amount of time that can be spent in inspiration.
Many devices set a TI min and a TI max.
The min is the least amount of time that a patient can inspire (before the breath is terminated).
The max is the most amount of time that the patient can inspire. After the TI max setting is reached, the machine automatically goes into expiration.
The range, example 0.1 to 2.0 seconds, means the patient's inspiration will be between that time frame. The patient's spontaneous breath can go into expiration at any time during that time frame. If the patient does not end the breath, then the TI max time is reached and the machine automatically stops inspiration and goes into expiration.
For many adult patients a TI max of 1.2 is comfortable. Please note that a default setting of 2 seconds is common on many devices. Every patient is different so it is important to find the proper setting.
If a large leak is present the machine may not sense the patient's attempt at ending inspiration. Therefore it's important that the TI max setting be set.
