Resmed I began CPAP Jan08 with a Dx of mild OSA,AHI 8.I did a home study for titration that resulted in a 7.2cm setting on CPAP.Over the next few months I regularly charted LED results from my Resmed S8 Elite. The AHI was usually less than 2 and leaks were low.I also used the SPO7500 oximeter and desaturations were few and minimal...91-92 range. However, I continued to have urinary frequency(nocturia) similar to my pre-treatment experience.Being a doc I knew something was wrong so I decided to spring for a Resmed S8 AutoSet11 since Medicare paid for the CPAP unit.My hunch was right on as the first and subsequent nights showed 95%tile pressures in the 8.5-9 range. The nocturia quit immediately. In retrospect the home study was flawed with high leaks and poor compliance.I set the APAP range to 6-12cm and I wonder why the high range should be lowered from 20cm. I assume the machine might erroneously detect a flow limitation and over respond.I guess the bottom line is that data can lull you into a false sense of confidence. Finally, I want to express my appreciation for this forum which has helped me so much.As a newbie to posting,I don't know how to show my equipment.. machine ,mask etc. ...help me puleese.

Thank you.
Re: Resmed AutoSet 11 success story
there is really no reason to lower the Maximum pressure on a modern day Auto. On the older units that could have been a problem, but the newer machines have greatly improved and there is not much risk of runaway pressure. That machine won't respond directly to an apnea over 10 cm pressure (only Flow Limitation and Snore).
On the Resmed machines, they always seem to score a higher HI than other machines so it is good to go by the individual AI and get that down to desired level, with that machine you can take AI down to .8 or even .4 and HI should rapidly fall. But your AHI of 2 on the Elite was nothing to worry about.
