The real challenge is getting safe therapy to those who most need it--ASAP!!!
Individuals are definitely responsible--they need to be identified, and LOCKED UP.
In fact an overseer can do just that. (S)he can make sure that they're not diverting resources to any kind of cover-up or incomplete remediation program. For instance, allowing the bad machines to remain out there to potentially be used by patients or be "harvested" for parts. Allowing the machines to be used for parts potentially means releasing the toxins when opening the case, or supplying contaminated parts.
