NEED A HIGHER CPAP PRESSURE???

Hello everyone!!

4 years ago I was diagnosed as having sleep apnea.. My doctor indicated me a CPAP with 9cm2 pressure, than my life got much better!!! I began to live as I never lived before... But unfortunately 10 months later I began to feel everything again (sleepy, tired, dizzy, grumpy e etc).. Since that time, I've been looking for a lot of doctor and each one says me one different thing: anxiety, insomnia, depression and give me a lot of medicines, but all of them work just 2 or 3 days and than I just have the side effects.. One month ago I went to a different doctor that said my CPAP pressure should be increased to 16cm!!!! I'm increasing 1 cm by week and now I'm already at 13cm2.

Need help with these questions:

1) It may happen that you have to increase the CPAP pressure from 9cm to 16 in just a few time? My weight is exactly the same of 4 year ago...
2) With my results (down are the report of Oscar), do you thing I'll get better with a 16cm pressure?
3) As I'm increasing the pressure, my mouth is getting dry every night.. Is it normal? What should I do?

you say you have oscar? if so, can you show us the data from that?

also, when you have time, could you please fill in your equipment profile? we don't all have the same equipment, so it will make a difference which one you have.

Put the machine in auto mode and start with the pressure back to min 9cm and have max at 20 and see what the machine wants.
You can't treat CA with your machine, they may be just pressure induced, if you didn't have them before. See what the reduced pressure does.

The clinical manual for your machine, is available on apneaboard.

No need to redo this one though.

Do you take any medications like some pain meds that might suppress your respiration???

I am not so sure that all those hyponeas are obstructive and if they aren't more pressure isn't going to help them.
Drop back to 9 cm and lets see what it shows....and turn Flex exhale relief off if you can...if you can't then try reducing it.

I would like to see some of that PB flagged breathing zoomed in though..
See this for some videos explaining how to zoom in and see the breath by breath flow rate.

This time frame that I circled would be a good one to zoom in on.

Thank you very much Pugsy!!

The only medication I take is venlafaxine in the morning and melatonin 5mg at night. I will try to drop back to 9 cm next night, than I send you the results. Just one question: the flex exhale relief could be related to the problem? I never tried before to turn it off.

Sometimes the drop in pressure when using exhale relief creates a situation with bilevel pressures can trigger central apneas...those CAs you see flagged.
Very small percentage of people who use exhale relief will have it happen...but need to rule out simple and easy fix first for all those centrals.

See if there is any reduction in the number of centrals with Flex exhale relief off.

Look at my last night report, when I increased the pressure to 14cm2!! My AIH got worse!!! And now with a lot of "OA"! Do you Know what it means?

PS; next night I'll try pressure 9.

I'm curious if your doctor even looked at your data before deciding that you needed more pressure? How did the doctor arrive at the idea that your pressure needed to be 16 cm. Was it just a guess???

Sleep varies, you need *more* pressure to prevent the OAs.

A one cm pressure change is pretty insignificant.

Don't make big changes based on a single night.

@bsgontijo - please do as Pugsy requested above. There's a very good reason she asked.A lot to be learned about your therapy by looking at the circled area. Heck, I learned something just from the fact that she chose this area.

The videos are extremely instructive. For one, you'll learn how to examine your events and understand whether or not they are real asleep events.

To follow up on djam’s message, just zoom in tighter so that the area Pugsy circled fills up the entire horizontal chart space.

I used my CPAP with pressure at 9cm2.. Also turned off the Flex exhale relief.. Last Night I had 6,6 AIH. What do you think I should do now? Still feeling tired...

How many times were you awake last night? I see at least one break in therapy where the machine was turned off and back on again so you were awake at that time. What about from 5:15 on? Awake or semi awake from that time on?

Refresh my memory please.....why are you using cpap fixed pressure mode?

Are you aware that in cpap mode the FL flagging is turned off?
